RESPONSE TO INFRACTION 50-566/80-14-01 AND 50-567/80-14-01
Infraction
As required by Criterion XI of Appendix B to 10 CFR 50, and implemented by
PSAR Section 17.1 A.17, " Test results shall be evaluated to assure that test
requirements have been satisfied."
Contrary to the above, thirteen percent of test cylinders for concrete
class 401.5 AFW have broken below the specified strength and no QCIR has
been issued nor has an evaluation of the questionable results been made.
Corrective Action Taken and Results Achieved
QCIR 51057 was written and an evaluation was performed by the Yellow Creek
materials engineer. The evaluation was conducted according ta General
Construction Specification G2, " Plain and Reinforced Concretc," This
evaluation disclosed that the mix designs for each affected strength class
required minor mix adjustments. In cddition, the evaluation indicated that
a procedural change was needed to ascertain that the cylinder test data
were being tracked and evaluated. The materials engineer made the
appropriate mix changes resulting in a lower failure rate which will bring
the overall percentage back within the acceptable 10 percent limit.
Finally, a review of the cancrete test dispositioned in QCIR 51057 revealed
that the design strength should be realized at a later than design age but
before the concrete is su)jected'to their design stresses.
Corrective Steps Which Will Be Taken to Avoid Further Noncompliance
To prevent further recurrence, QCI 209 (Concrete Strength Testing) will be
revised in order to clearly define the requirements for the materials
engineer to write a QCIR for each test that fails in accordance with
procedure CEP 15.01.
The materials engineer will be reinstructed in these procedures.
Date When Full Compliance Will Be Achieved
We expect to be in full compliance by December 1, 1980.
