Inadequate surveillance testing of containment pres-sure transmitters e

Conditions Prior to Occurrence: The unit was in Mode 1 with Power (MWT) = 2024 and and Load (Gross MWE) = 683 Description of Occurrence: At 1630 hours0.0189 days <br />0.453 hours <br />0.0027 weeks <br />6.20215e-4 months <br /> on November 17, 1980, the NRC determined that the station had not been performing all of the surveillance testing required by Technical Specification 3.3.2.1 Table 4.3-2 Note (2) on containment pressure trans-mitters. This note requires that the functional test shall include exercising the transmitter by applying either vacuum or pressure to the appropriate side of the transmitter. Therefore, all four containment pressure transmitters were declared inoperable, placing the station under the requirements of Limiting Condition of Operation 3.0.3.

This specifies that if a limiting condition for operation cannot be satisfied, the station would have to be in hot standby within one hour and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br /> unless corrective measures are completed.

At 1730 hours0.02 days <br />0.481 hours <br />0.00286 weeks <br />6.58265e-4 months <br />, NRR granted an emergency technical specification change which allowed the station to remain in operation.

Designation of Apparent Cause of Occurrence: The cause was a procedure deficiency in the Safety Features Actuation System (SFAS) Monthly ST 5031.01 Section 6.4 which tests the containment pressure transmitters. The test as written aimulates a pressure sig-nal at the SFAS cabinet. However, Technical Specification 3.3.2.1 Table 4.3-2 Note (2) requires the pressure transmitter to be tested by applying a vacuum or pressure to the pressure transmitter on a monthly interval. This is presently done in the SEAS Calibration Refueling ST 5031.03 but on a 18 month interval.

Analysis of Occurrence: Thare was no danger to the health and safety of the public or to station personnel. The three tests for proper calibration (ST 5031.03) that were run since initial criticality in 1977 showed that only one of the four trans-mitters in the 1978 test required recalibration. The test run in January 1980 and the test run immediately after this finding in November 1980 show that all four pressure transmitters were operable and holding proper calibration.

Corrective Action

At 1700 hours0.0197 days <br />0.472 hours <br />0.00281 weeks <br />6.4685e-4 months <br /> on November 17, 1980, Instrument and Control person-nel begar. testing the pressure transmitters under ST 5031.03. At 2300 hours0.0266 days <br />0.639 hours <br />0.0038 weeks <br />8.7515e-4 months <br />, all testing was complete, and the containment pressure transmitters were declared opera-ble. All were found to be in proper calibration.

ST 5031.03 will now be performed on a monthly basis in compliance with Technical Specification 3.3.2.1 Table 4.3-2 Note (2).

Failure Data: There hat = been no previous reports where the procedure contained the wrong surveillance f requency.
