h e (V Appendix A NOTICE OF VIOI.ATION Illinois Power Company Docket No. 50-461 Based on the results of the NRC inspection conducted September 2, 22-30, 1980, it appears that certain of your activities were not conducted in full compliance with NRC requirements as noted below.
This item is an infraction.
10 CFR 50 Appendix B, Criterion IX, states in part, " Measures shall be established to assure that special processes, including welding.......
are controlled and accomplished by qualified personnel using qualified procedures in accordance with applicable codes, standarda..."
Clinton PSAR, Paragraph 17.1.9.5 states, in part, " Inspections........
shall be specified and performed to assure and document satisfactory performance of the special process."
Baldwin Associates QA Manual, Section 9.4 (Requirements of Safety Related Items) states that, "Special processes shall be controlled through the use of procedures, checklists, or travelers."
Clinton PSAR, Paragraph 3.13.3.4.4 states, in part, "As a minimum, visual inspection of all weldments will be adopted to assure conformity of weldments..."
Baldwin Associates Technical Services Manual BTS 405, Section 9.1 states, in part, " Technical Services Inspectors shall document completed visual inspection of weldments......on traveler packet fore........"
Contrary to the above, approximately 2,000 temporary attachments such as scaffold brackets, gussets, and plates have been welded to the dryvell wall liner and the primary containment liner without the controls required for safety related equipment. Weldments were performed without traveler documentation control. QC Inspection and NDE were not performed or docu-mented.
