ABNORMAL OCCURRENCE REPORT REPORT NUMBER:
50-29/7h-h REPORT DATE:
July 11, 197h OCCURRE'!CE DATE:
July 2, 197h FACILITY:
Yankee Atomic Electric Company,, Rove, Massachusetts 01367 t
IDENTIFICATION OF OCCURRENCE:
Chlorides Exceeding Specifications in the Reactor Coolant System CONDITIONS PRIOR TO OCCURRENCE:
Refueling Shutdown, Shutdown Cooling and Purification Systems Operating,. Reactor-Coolant Temperature at 100 F.
DESCRIPTION OF OCCURRENCE During routine chemistry analysis of the reactor coolant, a chloride ion concentration of 0.1 PPM vas detected. This is a violation of Technical Specifications, Appendix A, Section B.2, referencing Section 106, Reactor Coolant Chemistry of the Final Hazards Summary Report which lists a chloride.
ion concentration limit of less than 0.1 PPM.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE Exhaustion of the purification system ion exchanger.
ANALYSIS OF OCCURRENCE During refueling shutdown normal purification is in service on the reactor coolant system. A mixed bed ion exchanger had been in service since the 4
start of refueling.
The bed indicated that it was spent on July 1, 197h.
i Remedial action was delayed for one day.
By this time'the chloride ion.
concentration reached 0.1 PP'<, thereby exceeding the Technical Specification -
limit of less than 0.1 PPi!.
CORRECTIVE ACTI_0_N The spent ion exchanger was replaced on the afternoon of July.2,.197h. At 0100 on July 3, 197h the chloride. concentration had decreased to less than S.1 PPM.
The Chemistry Department will change its method of chloride analysis from ASTM D512 Method A to ASTM.D512 Method C.
. Chlorides as lov as 0.02 PPM'can be detected using Method C while 0.1; PPM'is the lowest Method A can detect.
- This. change in method vP1 result in personnel _ noting trends at an early enough stage to take corrective action.
