Valve Surface Defects Prepare #. by:

E. R. Loibl, Actirig Sr. Reactor Inspector Date' A.

Date and Manner AEC was Informed:

In response to Region I questions concerning the valve wall thickness verification program (

Reference:

Region I letter to Yankee Atomic Electric Company dated February 7,1973 and Yankee Atomic Electric Company response dated March 7, 1973), the licensee informed Region j

I by telephone on March 13, 1973 that surface defects were discovered on the reactor coolant isolation valves.

B.

Description of Particular Event or Circumstance:

j The licensee stated that during preliminary surface cleanup prepara-i tions, being done prior to adding veld overlay to the reactor coolant I

I isolation valves, nondestructive testing (liquid penetrant) re-vealed surface defects on all eight of the valves. Most of the

~

crack like-defects described by the licensee as being discontinuities, I

.were stated to be quite shallow (approximately.030") and the maximum defect was approximately.120" deep and 1/2" long. All

.j-of the defects were stated to have been removed by surface grinding.

)

Removal of the defects was stated to have been verified by non-

~

l destructive testing (liquid penetrant).

The work of applying weld overlay (to meet minimum wall thickness requirements) was stated by the licensee to have been completed.

C.

Action by Licensee:

}

A written report to the AEC 'will be submitted by the licensee 1

addressing the defects found and corrective actions taken.
