Dear Mr. Keppler:

On July 7, 1980, Mr. C. E. Chmielewski of Public Service Company of Indiana, Inc., (PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55(e).

Results of a Westinghouse quality verification package review as performed using Special Process Procedure Mo. 7 to verify the quality of previous work indicates 46 nonconformances out of a sample size of 49 document packages. The predominate deficiency is missing documentation. Until records become available that can substantiate that quality related activities were properly performed, the quality of the related equipment could be considered indeterminate.

Preliminary contact with the Westinghouse Corporation indicates that records are available and Westinghouse is obtaining the records at the present time. When these records are provided to PSI, an evaluation will be made of their adequacy and effect on the performance of the related equipment. Your office will then be notified of the results of this evaluation.

This letter is intended to fulfill the requirements of an interim report as defined in 10 CFR 50.55(e). A final report will be filed on or before December 15, 1980. If you have any questions on this report, please feel free to contact me.
