Docket No. 99900109/80-01 NOTICE OF DEVIATION Based _on the results of an NRC inspection conducted on March 24-28, 1980, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:

Criterion V of Appendix B to 10 CFR 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Deviations from these requirements are as follows:

A.

Pittsburgh - Des Moines' (PDMs) Corporate QA Manual section 13, sub-paragraph 13.1.1, states:

4 "The controlled manufacturing and construction process shall be detailed and documented by using the Fabrication Check List (FCL) (Form 18011).

The operation steps shall be written in sufficient detail to assure control and traceability of all work and items. Mandatory fabrication sequences specified on drawings and in procedures shall be included on the FCL."

-Contrary to the above, temporary attachments were being attached and removed without being documented on the FCL in sufficient details to assure control and traceability of the work and items.

B.

. PDM's Corporate QA Manual, section 13.2.4, welding Procedure Control States:

"The Welding Supervisor shall assure that the WPSs are available to the welders or welding operators, and that the welders and welding operators understand and follow the parameters outlined in the WPS.

QA shall sufficiently monitor welding operations to assure conformance to procedures and specifications. When such monitoring is performed it shall be documented on the FCL or on a monitoring record to demon-strate compliance with this monitoring requirement."

Contrary to the above, two (2) welders were observed welding out of the 4

i electrical requirements established in Welding Procedures70-110 and 7L47.
