Dear Mk. Grier:

On June 24, 1980, in accordance with 10CFR50.55(e), we reported verbally to Region 1, a deficiency with 24 volt D.C. power sup-plies associated with Bailey Controls Company's Electronic Ana-log Instrumentation.

This letter serves as our 30-day written report of this deficiency.

Description of Deficiency Bailey Controls Com7any, a supplier of Electronic Analog Instru-mentation for the Shoreham Nuclear Power Station, informed us on March 21, 1980 of a potential deviation associated with ITT North Electric Power Supplies (Bailey Controls Company, Part Numbers 8080B02P0008 through and including 8080B02P0012).

These are 24 volt power supplies used in the 4 to 20 milliamp control and logic loops of the HPCI and RCIC lube oil coolers, control room air conditioning, RBSVS and various chillers, all of which are Q.A. Category I.

The letter from Bailey identified the source of the potential deviation as the AC stability capacitor C-1, which in some instances had contamination which resulted in overheating of the capacitor causing swelling and leakage or rupturing of the capacitor case.

The resultant effect on power supply operation was said to be varied and not totally predictable.

A customer bulletin was supplied with the Bailey Controls letter and tests to datermine if'the capacitors were indeed deficient were cutlined.

July 24, 1980 SNRC-487 Page 2 The Shoreham equipment delivered to Long Island Lighting Company contained ten (10) power supplies; six (6) Part No. 8080B02P0008 and four (4) Part No. 8080B02P0010.

A test was performed May 12, 1980 with a Bailey Controls Company field representative present and seven (7) power supplies were identified to have potentially defective capacitors.

An analysis of application in the Shoreham control loops was made and the possible impact of various failure modes of the aower supplies identified.

A determination was made by Stone & Weister Engineering Corporation on June 23, 1980 that cooling for safety related equipment and temperature control of critical plant areas could deteriorate with resultant adverse ef-fact on safety systems.

Notification was given to the NRC on June 24, 1980.

Corrective Action Bailey Controls Company was notified of our determination that a 10CFR50.55(e) reportable deficiency exists and a decision was made to replace the defective ca represenective replaced six (6) pacitors in the field.

A Bailey capacitors and tested the as-sociated power supplies during the week of July 14, 1980 and will return within one month to complete the replacement and testing.

A follow-up report will be submitted within 90 days to document the completion of corrective action and steps to prevent reoccurrence.
