Dear Sir:

This letter is in response to your Inspection Report dated June 27, 1980 which contained two items of noncompliance related to approval of test procedures and qualification of testing personnel. The following corrective action response is provided for these items of noncompliance:

A.

Infraction: Section 11.3.1 of the Qualify Assurance Program for Operations stacea, in pa r t, that, " Test procedures involv-ing quality-related items are reviewed by the Plant Review Board (PRB) and approved by the Plant General !!anager."

Contrary to this requirement, the test procedure used by Licensee personnel on May 20-23, 1980 to test decouple switch breakers had not been reviewed by the PRB and approved by the General !!anager.

Response: The test procedure, Special Plant Test #58, for testing the decouple switch modifications was reviewed by the PRB and approved by the General !!anager on June 24, 1980.

l To clarify the QA Program requirements on testing, an Interim l

Change 80-T1, to the Nuclear Projects QA Program for Opera-tions was approved on July 14, 1980. This change addresses and differentiates between installation checks and testing.

Plant procedures which relate to approval of test procedures will be revised by September 15, 1980 to more specifically address the type of testing requiring review of the PRB and approval of the General flanager.

B.

Deficiency:

Section 11.3.2 of the Quality Assurance Program for Operations states, in part, that, " Tests are conducted

WWMW Mr. R. H. Engelken July 17, 1980 Page 2 and documented on the test data sheets by personnel qualified and certified in accordance with written procedures conforming to ANSI N45.2.6."

Quality Assurance Procedure QAP-11-2, Sec-tion II.E, further states, in part, that " Records supporting personnel qualification shall be... retained as QA records..."

Contrary to this requirement, the Licensee did not. have records to establish required tester qualification for (1) the indivi-dual performing tests of safety-related circuit breakers on Iby 20-23,1980; and (2) individuals who performed tests of pressurizer code safety valves on }by 2, 1977; March 19, 1978; and April 30, 1979.

Response: On May 21, 1980, a Quality Notice, #8014, was ini-tinted to docuraent that maintenance personnel who had performed testing of main steam and pressurizer code safety valves were not qualified testers. thintenance personnel who were involved in testing activities were evaluated and determined to be qualified although not certified.

Subsequently, these pe r-sonnel were certified as Level II Testers on May 29, 1980 and the Quality Notice closed. The corrective action taken for the above Quality Notice will adequately resolve this item of noncompliance.
