This refers to the routine inspection conducted by Mr. C. T. Oberg and Dr. W. J. Adam of this office on July 21-23, 1980, of activities author-ized by NRC Byproduct Material Licenses No. 12-00088-06, No. 12-00088-08, No. 12-00088-09 and Special Nuclear Material License No. SNM-1552 and to the discussion of our findings with you and Mr. P. C. Philip at the conclusion of the inspection.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspection consisted of a selective examination of pro-cedures and representative records, observations, independent measure-ments, and inte. views with personnel.
During this inspection, certain of your activities appeared to be in non-compliance with NRC requirements, as described in the enclosed Appendix A.
On the date of this inspection, the inspectors observed that your AECL Gammabeam irradiator was left unattended while the key which activates the unit was left in the control console. This console is mounted in an unrestricted hallway.
In your reply to this letter, please state how you will improve the security procedures for this unit such that unauthorized individuals will be prevented from activating it.
During the exit interview with you, our inspectors expressed concern for an apparent lack of management control, particularly within the nuclear medicir.e program.
In your reply to this letter, please state what measures you will initiate in order to ensure that the daily and routine commitments l
are fulfilled. Please refer to Appendix B for guidance regarding manage-ment control.
This notice is sent to you pursuant to the provisions of Section 2.201 of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations.
Section 2.201 requires you to submit to this office within twenty days of your receipt of this notice a written statement or expla-nation in reply, including for each item of noncompliance: (1) corrective action taken and tne results achieved; (2) corrective action to be taken acosesa77/
