- 1. Provide a diagram which illustrates the physical relation between the reactor core, the fuel transfer canal, the spent fuel storage pool and the set down, receiving or storage areas for any heavy loads moved on the refueling floor.
RESPONSE
Drawings C-429 and C-430 are included which show the relationship between the Reactor, the Steam Dryer and Separator Storage Pool, the Spent Fuel Storage Pool and the Cask Storage Pool.
- 2. Provide a list of all objects that are required to be moved over the reactor core (during refueling), or the spent fuel storage pool. For each object listed, provide its approximate weight and size, a diagram of the movement path utilized (including carrying height) and the fre-quency of movement.

RESPONSE
Iowa Electric, at this time, does not intend to move fuel elements from the SFP for any reason in the near future. Therefore, the dimensions and weights of the Spent Fuel Casks have not as yet been determined. This information will be provided prior to the first movement of a diipping cask.
'4 Identify any heavy load or cask drop analyses performed to date for your facility. Provide a copy of all such analyses not previously sub-mitted to the NRC staff.

Please refer to page 16 of report, " Design and Safety Evaluation for Replacement of Spent Fuel Storage Racks", transmitted via Iowa Electric letter dated Octobcr 13,1977 (Rev.1 dated June 6,1978) from Mr. Lee Liu to Mr. Edson Case.
- 5. Itentify any heavy loads that are carried over equipment required for the safe shutdown of a plant that is operating at the time the load is moved. Identify what equipment could be affected in the event of a heavy load handling accident (piping, cabling, pumps, etc.) and discuss the feasibility of such an accident affecting this equipment. Describe the basis for your conclusions.
RESPONSE
It is presently intended that 'P.ere will be no heavy loads carried over operating equipment required for the e .fe shutdown of the DAEC.
- 6. If heavy loads are required to be co ried over the spent fuel storage pool or fuel transfer canal at your facility, discuss the feasibility of a handling accident which could result in water leakage severe enough to ur. cover the spent fuel. Describe the basis for your conclusions.

Also, refer to Item No. 2 e this report which lists all of the material which is carried over '.ne Spent Fuel Pool (SFP). For-the rack replacement procedure, please refer to Item No. 7 of the enclosure to Iowa Electric letter dated MarcL 14, 1978 from Mr. Lee Liu to Mr. Edson Case. That letter transmitted additional information requested on the replacement of spent fuel storage racks.
- 7. Describe any design features of your facility which affect the potential for a heavy load handling accident involving spent fuel, e.g., utilization of a single failure-proof crane.
RESPONSE
Refer to DAEC FSAR Section G.13 for a description of and the use of cranes in the Spent Fuel Pool area. The Cask Storage Pool is physically separatcd from the Spent Fuel Pool (SFP) which eliminates the necessity of the heavy cask load being moved over the SFP.
- 8. Provide copies of all procedures currently in effect at your facility for the movement of heavy loads over the reactor core during rofueling, the spent fuel storage pool, or equipment required for the safe shutdown of a plant that is operating at the time the move occurs.
RESPONSE
A copy of the DAEC refueling procedure is included.
- 9. Discuss the degree to which your facility complies with the eight (8) regulatory positions delineated in Regulatory Guide 1.13 (Revision 1, December,1975) regarding Spent Fuel Storage Facility Design Basis.

Because of the tre=endcus veight of the shield blocks, they must be arranged so as not to exceed the design floor loadings as they are removed. Refuel-ing Equip =ent Storage drawings show where each shield block should be placed during refueling. This layout is recen= ended in light of floor loadings, 1 maxi =u= crane travel, and efficiency of refueling operations. These three factors must each be carefully studied prior to deviating from the reco== ended layout, if such need arises. 1 I The order of re= oval is as follows.

B. VENT GAS FRCM REACTOR HEAD As the reactor is depressurized, gases which has been dissolved in the water will escape, forming a. bubble of noncondensible gases in the vessel head. To preclude the escape to the atmosphere of these potentially radioactive gases as the head is lifted, the following ventin5 procedure should be executed:
- 1. Following resetor shutdown, co==ence vessel cooldown and pressure blevdown as no. 1 C1 2.
V Raise the reactor vnter level to the vessel flange using the most feasible =ethed availrble at that time. CAUTIO:i: Tollov vessel tenperatures during this step. Be particularly careful as the water level reaches the flance so that ther=al limits are not exceeded.
- 3. After reactor pressure reaches atmospheric, open the reactor head vent valves. This vill vent any gases present in the head to the dryvell i

- k. Continue to raise the water level very slowly to preclude pressurin-ing the reactor. Note the level en the vessel flooding level indicator.
i When that indicator reads upscale, watch for a change in the Dryvell Equipment Drain Su=p Flov Recorder. (This vill indicate that all gas has been forced out of the vessel head and that water is passing through the vent line.)
- 5 Terminate the addition of water to the vessel and reject vater to lover the level to below the main stea= lines.
- 6. close vent valves in preparation for MsI7 air leakage testing.

The dryvell head is held in place with 192 bolts. The end of each bolt is leveled so as to direct it squarely into its mating thread. The bolt end is slotted so that when removed, the bolt can be lifted and set into a re-tainer ring, held in place by the slot. A strongback is provided for lifting the k0 ten dryvell head. e s The drvvell is de-inerted prior 'to removing the head. As a precaution, the oxygen concentration in the reactor well should be measured as the head is lifted. The head should be located on the refueling floor as shown by the equipment storige drawing. 4 D v . III-3

3 Vessel Head Eeroval The.followin5 are general instructions on the renoval of the reactor pressure vessel head. For more details consult General Electric manual ( _ titled: Operation and Maintenance Instructions - Reactor Assemb3y - GEF-166kO.
- CAUTIO:i: Do not loosen closure studs until the water temperature in the vessel is below 200 F and.the pressure is reduced to O psiS . The minimum vessel netal te=perature for unbolting is-100 F. The upper vessel and closure head shall not be cooled belov LOO OF at any time when bolted.
- a. Using the reactor building main crane and stud tensioner assembly, loosen the vessel head studs in proper sequence.

- b. Using-the nut vrench and nut handling tool, remove the head nuts and washers. Store in th'e head nut and washer rack. !
NOTE: The. studs, nuts, and washer sets are numbered serially'in accordance with the hole location in which they are installed. In storing the nuts and vashers, keep matched sets together in the same racks.
- c. Remove the studs in line with the refueling slot. Proceed as follows:
(1) Renove the threaded plug from the top of each stud. (2) If required, use the stud vrench to loosen the studs. 1 l (3) Thread the head stud handling tool into the top of each stud I l in sequence. Unthread the stud from the flange, and after the threads are free, lift the stud out of the vessel flange using
, the reactor building auxiliary crane. .
(h) Store the studs in the head stud rack.
- d. Using the reactor building auxiliary crane, transport each head nut and vasher rack and the head stud rack to the storage area on the refueling floor.

as follows: (1) Verify that the two centerplate hook pins on the head strongback are retracted onto the pin supports. (2) Lower the, crane hook'into the strongback hook base. NOTF.: The semicircular' groove in each hook pin must be facing downward prior to the performance of Steps f.(3) and f.(h). (3) Using the pin insertion tool, push one of the two hook pins

(h) Repeat Step f.(3) for the second hook pin.
- g. Operate the crane as required to raise the head strongback off the operating floor approxinately h feet. ,
- h. At each of the four lifting ams on the strongback, move the vessel turnbuckle assembly from the stowed position to the downward-handing lifting position.

-- The vessel head turnbuckle is the inboard of the two turn-buckle assemblies at the end of the lifting arm. The out-board assembly is used to transport the dryuell head and must remain stowed on the lifting arm during this procedure.

- 1. With all personnel clear of the reactor well, carefully lift the strengback and vessel- head off the vessel flange.
- m. Carefully raise the vessel head " cut of the reactor *.:c11, and trans-port it to the head holding pedestals on the operating floor.

I i NOTE: Prior to lowering the vessel head onto the pedestals v (see Step n), lay new vessel head flange 0-rings on the operating floor within the pedestals to facilitate sub-sequent installation into the vessel head flange (see, Step t).
- n. Carefully lover the vessel head and seat it on the head holding pedestals.
- o. With the vessel head properly seated, lover the crane hook as re-quired to slacke's the cables.
NOTE: If the strongback is to remain attached to the vessel head until the head is reinstalled, omit Steps p. and q. below. OO
- p. Disasse=ble the turnbuckles from the shackles on the vessel head lifting lugs. Stow the turnbuckle asse=blies in their stowed posi-tions on the head strongback lifting arms.
4 q. Operate the crane to carefully raise the head stron6back, and r.ove it to its stora6e area on the refueling floor. 4
- r. Slacken the crane cable. Disengage the ball-lock pins from the
; ends of the head strongback hock" pins, and using the pin insertion tool, push the hook pins onto the pin supports to free the crane hook from the hook box. }

() s. Raise the hook out of the hook box. NOTE: Im=ediately following RPV head re= oval, install the flange seal surface protector.
- t. With the vessel' head resting on the head holding pedestals, proceed as follows to replace the vessel head flange 0-rings.
(1) Remove the old vessel head flange 0-rings, and thoroughly clean the flange 0-ring grooves. Do not use tools which vill scratch or gouge the grooves. (2) Inspect the new O-rings for defects. NOTE: To prevent danage, the 0-rings must be handled in a horizontal fully supported position. Personnel handling 0-rings must wear soft lint free gloves to prevent tarnishing the silver plated surface. l (3) Place the new O-rings in the vessel head flange grooves, using the retainer clips provided. k) \s 111-10 l

- 1. Prerequisites
- a. The RPV head must be removed and stored on the operatin6 floor.
- b. The dryer -seperator canal shield blocks must be removed fro = be-tween the reactor well'and the dryer-separator stora6e pool and stored on the refueling floor,
- c. The reactor vell (and dryer-separator pool) must be drained to the level of the RFY flange.

-sling are required for steam dryer assembly removal and installation.
- 3. Stes: D:fer Assently Re= oval Proceed as follows to remove the steam dryer assembly: -
- a. Attach the reactor building main crane hook to the dryer-separator sling as follows:
(1) Verify that the two hook pins on the sling hook box are re-tracted onto the pin supports.

- b. Operate the crane to transport the slin6 over the reactor well.
- c. With one technician on the refueling floor holding the sling control
, box, and with the sling suspended over the reactor well, test-operate the slin5 as follows to demonstrate operational readiness.
(1)' Connect' the control box to a source of 100 psi service air from the refueling floor.
- (2) With the control box control valve in the, IUSERT position, check ;
the four pressure cauge's on the control box, nnd verify that all four air cylinders' on the sling are pressurized. 9-24-74 III-12 Rev. 2

(3) Observe the four pressure gauges on the control box, and verify that the four lifting pins have inserted through the sockets. (4) Move the control box control valve to the RETRACT position. Check the pressure gauges, and verify that control pressure has been relieved. Observe the pressure gauges and check that the four lifting pins have retracted out of the sockets.
- d. Operate the main reactor building crane to. lover.the sling down into the reactor. -
- e. From the reactor well, nanually guide the sling so that the four sockets (Figure 33, Section II) seat over the steam dryer lifting d(~m lugs.
- f. Move the control box control valve to the INSERT position to insert the four lifting' pins through the sockets and lifting lugs. Verify that the pins have inserted through the sockets.
CAUTIOR: EIvery tine the dryer-separator sling is used to lift the dryer assembly, the load distribution at the four lifting points should be checked to insure that the load is dis-tributed equally to the four turnbuckles on the dryer-separator sling. An unequal load could result in severe equipment damage and possible injury to personnel. l h

7 L . 2S g. Operate the crane to carefully raise the dryer assembly out of 'the L.) reactor vessel until the bottom edge of the dryer assembly skirt is in position to clear the curb of the dryer-separator canal.

Transport the dryer assembly to its storage area at the far end of the dryer-separator pool.
- i. Operate the crane to carefully lover the dryer assembly onto the

floor of the dryer-separator pool. Slachen the sling cables. WARIIING RADIATION HAZARD: The steam dryer assembly must be kept vet to prevent airborne contamination until such time as the dryer-separator pool is flooded.

- j. Move the cor. trol box control valve to RETRACT. Check tht.: all four pins have retracted by verifying that all four pressure gauges indicate zero pressure. '
, k. Operate the crane to carefully lift the sling, and verify that the sling has disengaged from the dryer assembly lifting lugs.
NOTE: The shroud head and steam separator assembly is nora- lly removed following removal of the steam dryer assembly. In this event, disregard Steps 1 and m. - m. Slacken the crane cable. Using the pin insertion tool, push the lock pins onto the pin supports onthe hook box to free the crane book.

PLUG MAIN STEAM LINES For additional s details, refer to General Electric Instruction GEK-331013 l contain-d in DAEC Operation and Maintenance Manual, GEK-9692; Volu=e VI. l The steam line plug is used with the reactor building auxiliary hoist, a manually operated chain hoist (1/2 ton), the stes: line plug installation t erg ! \- i -s ; tool (See Figure 53, section II), and an actuating pole with an L-hoch l . I ' attach =ent. A line is required to lash the plug to a vessel stud after it
- has been installed.
I I To install the steam line plug, perform the following steps: l l
- 1. Place the hanger plate assenbly (air control panel) on the safety l
railing where it will not interfere with-other operations.
-2. Connect service air (100 psig) to the air control panel quick connect stem. Insure that the regulator and globe valve are closed.

- 3. Connect 50-foot hose between the air control panel bottom connector and the connector on the line at the steam line plug vent pipe.
'k. Use the hand hoist to suspend the steam line plug installation tool from the hook of the auxiliary hoist. ~
- 5. Latch the stezn line plug in the installation tool. ;
- 6. Using the auxiliary hoist, position the steam line plug adjacent to the reactor vessel no::le. .
7 . While a man reaching over the studs is guiding the plug, lower it such that it slides along the vessel vall until it slips into the nochle.
- 8. Use the hand hoist to adjust elevation for vertical clearance of the plus in the no::le, and move the auxilia'ry. hoist horizontally to insert the plug farther into the no::le unt'il the 0-ring touches the reactor vessel vall.
9 Adjust the regulator at the air control panel for 30 pais.
- 10. Lash the handle of the ' vent pipe to the nearest stud with tie line.
- 11. Using an actuating pole with L-hook, release the plug from the installa-tion tool.

I 12. Using the auxiliary hoist, move the installation tool horisontal17 away fro = the plug and then raise it out of the reactor vessel. 13 Using the same procedure, install the retaining stea= line plugs.
- 14. Raise the reactor vessel water level up to the vessel flange.
15 Recove the vent cap on the vent pipe of the steam line plug.
- 16. Drain the stea line.
UCTE: The pressure differential between vessel and stea= line _, vill ec:plete the insertion of the plug and ccepress the 0-ring until the square face plate of the plug is approxi-tately 1/k inch from the vessel vall. 17 Using the regclator and globe velve at the 'a'ir control panel, bleed all air frc: the inflatable seal. The seal' vill reposition. e
- 18. At the air control panel, insure that the globe valve is closed, and adjust the air supply for 30 psig.

The fuel pool gates are designed to be held in place by their own weight. The hangers counted on the pool valls are beveled such that the weight of the gate forces it closer to the fuel pool vall, compressing the gate gasket. In addition, the water pressure on the pool side (with the reactor well drained) further compresses those gaskets. Also, two bolts on the top of each gate assist in compressing the gasket. The gates are removed by the following steps. Refer to Figure 59, Section II. l' . Flood the reactor well to equalize the pressure across the gates.
- 2. Connect slings frem the two lugs on the top of the south gate to the reacter building crane; raise the crane to recove slack cable.

. During refwling, the reactivity of the core is being altered. 'Certain ~
Id interlocks and refueling procedures assure that inadvutent criticality does not occur. When the mode switch is in the REFUEL position, as
- required b;- the Technical Specifications, interlocks prevent the Refueling Platform from being moved over the core if a control rod is vitlidrawn and fuel is on a hoist. Also, if the refueling platform is over the core with fuel on a hoist, control rod motion'is blocked by the interlocks. With the mode switch in the REFUEL position, only one control rod can be withdrawn.
The Source Range Monitors (SP24s) are provided to monitor the core during periods of shutdown and to guide the operator during refueling operations and startup. By requiring two operable SRMs, one in and one adjacent to any core quad ant inere fuel or control rods are being =oved, the Technical O Specificatiens assure adequate monitoring of that quadrant during alterations. The Technical Specifications require a mini =u= response of 3 counts per l second on the operable SP24 channels, to assure that neutron flux is being i monitored.
