Viciatien Facility Technical Specification 6.8.3 states: "Te=perary changes to precedures...=ay be made, provided" a)

Centrary to the abcVe, a change to surveillance prece hre F 19 "Startup Channels Cperational Check" was made en :ieve=ter 26, 1980 withcut the required approvals, decu=entatice and subsequent PRC review.
Res;cnse to Violation Test precedure TR-19 is utilized by the Instrument and control Depart =ent during refueling cutages as part of the preventive maintenance program for the startup neutron monitors. During the conduct of this test on Nove=ber 26, 1980, a minimum count rate using a protable neutron scuree was not achieved prior to checking voltage and discrimi':ator settings of the startup neutren
=enitoring equip =ent.
The deficiency in achieving the count rate was dis-cussed by the technician and his supervisor and criteria specified in the test was evaluated verbally as being unnecessarily restrictive. The ec=pleted precedure was reviewed and filed without dispositioning the deficiency in writing or changing the criteria by the approved =ethcds.
