Docket No. 50.382/80-26 Appendix A NOTICE OF VIOLATION Based on the results of the NRC inspection conducted during the period October 7-10, 1980, it appears that certain of your activities were not conducted in full compliance with the conditions of your NRC Construction Permit No. CPPR-103 as indicated below:

affecting quality shall be prescribed by documented instructions, procedures,.or drawings of a type 4.,ropriate to the circumstances, and shall be accomplished in accorosace with these instructions, procedures, or drawings.
The Mercury Company Procedure SP-656, " Fabrication of Local Instru-ment Piping and Tubing Assemblies," Revision 3, dated November 15, 1978, paragraph 6.3.7 states, " Plug all open ended valves with plastic caps or. equal.",
Contrary to the above:
On October 9,1980, during the IE inspector's tracking of instrument sensor tubing runs, the IE inspector observed six installed 1/2 inch needle valves that were open and unprotected. Two of these valves were identified as safety-related valves (Item 3 on Mercury Company Drawing 151-T-014-C, Rev. 0) at the 46 foot elevation in the Reactor Containment Building.
This is an infraction.
