I, Appendix A NOTICE OF VIOLATION i

Based on the inspection conducted on August 18-22, 25-29, September 2-5, 8-12, and 15-19, 1980, it appears that certain of your activities were in noncompliance with NRC requirements as noted below. This item is a noncompliance of the infraction level.
10 CFR 50 Appendix B Section VI requires that measures established to control the issuance of documents shall assure that documents, such as procedures, are used at the location where the prescribed activity is perfo rmed.
T.S. 6.8.1 requires that the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, November, 1972 shall be implemented.
Appendix A of Regulatory Guide 1.33, November,1972 includes procedures for startup, operation and shutdown of safety related systems. The Makeup and Purification System is a safety related system.
On July 3, 1980, Revision 10 of Procedure SP 1104.02, " Makeup and Purifi-cation System," was -approved and distributed.
Contrary to the above, on September 17, 1980, the inspector observed that the procedure being used by the control room operators to align the makeup and purification system was revision 9, not the latest approved revision (Rev. 10).
