- 1. Introduction Non-normalized one-half inch plate materials (SA-516 Gr. 70) purchased without Charpy impact testing were used in fabrication of ASME Code stamped personnel
' air locks for the Catawba Nuclear Station. The personnel locks, therefore, did not meet the specified requirements of the Catawba PSAR, Section 3.8.2.1 (2). This final report is being filed as required'by 10 CFR 50.55(e) to document this deficiency and to provide Duke Power Company's evaluation and the corrective action taken. A possible deficiency was first discovered by W. J. Woolley Company in November, 1979 (Attachment 6.1). A review, by the parties involved, of the material documentation including interpretive inquiries to ASME indicated that a deficiency did exist, applicable to Catawba personnel locks, Units 1 and 2 (Serial Numbers 32360, 32385, 32290 and 32335). Duke Power Company reported the deficiency to the NRC, Region II, as a 10 CFR 50.55(e) item on July 11, 1980. An interim report (No. 50 413-414/80-10) was submitted in August,1980.
- 2. Background Information ,
The four (4) personnel air locks for the Catawba Nuclear Station were designed by the W. J. Woolley Company of River Forest, Illinois. This company also supervised the, fabrication of the locks by their subcontractor, Lamco Industries, Inc. of El Cajon, California. -The general arrangement of the personnel locks is the same for all locks and is shown on drawing 32360 (Attachment No. 6.2). One-half inch SA-516 Grade 70 plate material was used in fabrication of the lock barrel, bulkheads, , stiffeners, and miscellaneous attachment supports and mounting plates. Cn November 16, 1979, Duke ' Power Company was informed by telecon (and follow-up letter dated November 20,1979) from W. J. Woolley Company of a possible ASME Code violation concerning this material. Since-that time, the problem has been under investigation by Duke Power Company, W. J. .Woolley Company, Lamco Industries, Inc. and Hartford Steam Boiler, the Authorized Inspection Agency. During the cour.- of the investigation, an apparent conflict was noted between paragraphs NE-2121(c) and NE-2311(a) of Section' III and the ASME material specification. Accordingly, an inquiry was sent to the ASME for a code interpretation. The response received indicated that the material :in question did deviate: from the ASME Code requirements,
. thus resulting in a 50.55(e) deficiency (See Attachments 6.5, 6.6 and 6.7).

Descriotion of Deficiency One-half inch ASME SA-516 Grade 70 PVQ carbon steel plate used in the fabrication of Catawba personnel locks S/N 32350, 32385, 32290 and 32335 was purchased-by Lamco Industries, Inc. in violation of ASME Section III, Subsection NE, Paragraph 2121.2, 1971 Edition, Summer of 1972 Addenda.
This deficiency is noted on W. J. Woolley Company Corrective Action Request Number 1047-1, Rev. 3 (attachment 6.3). The air locks have been tagged at the jobsite in accordance with Duke Power Company Catawba Nonconforming Item Report S/N 7999 (Attachment 6.4).
- 4. Safety Imolications The material in question was not normalized or Charpy imoact tested.
Normalizing of material is done primarily to improve incact properties and enhance ductility. For non-normalized material, Charpy impact testing assures minimum requirements are met. This is particularly important fo low temperature applications. Examination of the maximum stress levels for the Catawba personnel locks indicates that they are well within the elastic range for all loading condi tions. In addition, steady state thermal analyses of the Reactor Building complex indicate that the minimum personnel lock temperature will be above 480F, Therefore, actual design conditions would preclude the need for normalizing
- or Charpy testing except to meet an ASME Code requirement. 'If this deficiency had gone. undetected, public safety would not have been jeopardized.
E. Corrective Action The personnel locks were-fabricated using non-normalized one-half inch plate material from 20 slabs taken from 8 different heats. This material
-is acceptable, however, to meet the requirements of paragraph NE-2121(c) of the ASME-Code, Charpy impact tests are required. A Charpy impact testing program 'for these materials was developed and has been completed.

Report No. SD 413-414/80-10 March 31, 1981 Page 3 Some plate material common to personnel locks being fabricated for another utility was available in the fabricator's shop. In addition, some excess plate material used in the fabrication of the Catawba personnel locks was also available. Samples from this available material were Charpy impact tested in accordance with the requirements of ASME Section III, Subsection NE, Paragraph 2351, 1971 Edition, Summer of 1972 Addenda. This testing was done at Durkee Testing Laboratories, Gardena, California, and was witnessed by W. J. Woolley Quality As:urance personnel. For the remaining slabs, samples were secured from the personnel locks at the construction site. These samples were Charpy impact tested at Law Engineering Testing Company in Charlotte, NC and Atlanta, GA. Charpy testing and sample removal was witnessed by W. J. Woolley Co. and Duke Power Co. Quality Assurance personnel. The following infomation is included as documentation for the Charpy impact testing program: (a) Attachment 6.8 - W. J. Woolley Co. letters dated August 7 and 8,1980 concerning scope of required site sampling, testing and restoration of locks to full ASME Code compliance. ! (b) Attachment 6.9 - Summary table of mechanical and impact test properties for identified nonconforming material. (c) Attachment 6.10 - W. J. Woolley Co. sample removal procedure 1047-1A, Rev.- 2, including sketches of actual sample locations. (d) Attachment 6.11 - Certified Charpy impact test reports. The results of-the Charpy testing program indicate that all material previously identified as being deficient is now in full compliance with , applicable ASME Code requirements and the Owner's Design Specification and l Safety Analysis Reports. - l l ihe personnel locks will be fully restored to their original state prior

This work including materials, welding, NDE inspection and retesting, as required, will be done in accordance with the requirements i of Subsection NE, Section III of the ASME Code and under the provisions of l Section XI, including the Owner's site Quality Assurance Program for ASME Code work. All work will be inspected and approved by the Owner's Quality Assurance personnel and the site ASME Authorized Nuclear Inspector. .The restoration work will be completed by December 31, 1982.

Catawba Personnel Airlocks Centlenen: This letter is to confirn our telecon of November 16, 1979 concerning a possible reportable item to the N.R.C. Our fabricator, Lacco Industries Inc., purchased one-half inch plate which we believe was received in a non-normalized condition. This material was ordered without the Charpy testing requirenent per NE2311 of the ASME. Paragraph NE2121(b) recuires in excess of one quarter inch that is not Charpy tested to be normalized, fully killed and fine grained. All conditions except the nornalizing have been met. , The W. J. Woolley Company is investigating steps that can be taken to establish the acceptability of this material. Please find attached a photocopy of an answer to an inquiry concerning SA-516 material, According to the inquiry fiE-2121(c) allows that the material (SA-516) either be normalized or have impact testing performed which ceets the acceptance criteria of flE-2300. In order for the Personnel Airfocks in question for the Catawba tiuclear Power Plant to meet the requirements of the ASME Code the material of the lock would have to be either normalized or perfom impact testing as required. 'Therefore, the material used in the fabrication of the Personnel Airlocks must be impact tested if not normalized. An alternative would be to remove the ASME Code Stamping and furnish a ncn-code Data Report in lieu of the ASME Code Data Report if this would be acceptable to the owner. , If you have any further questions on the above, please do not hesitate to contact me. . With reference to Corrective Action Request 1047-1 Rev. 2. We have submitted our proposed disposition to our ANI (Hartford Steam Boiler Inspection Agency):
- They have reviewed and. submitted an inquiry to the ASME. , Please find the res-ponse enclosed.
It is the intent of the Woolley Company to meet the applicable require =ents based on the ASME Code interpretation. Therefore, we will have to take actual physical specimens for testing from the hardware. 1047-1A Rev. 1 Dated 3/23/81 SCOPE: Remove designated specimens from the plate material to identify heat and slab numbers so that Chcrpy V Notch Testing may be performed; to assure that the material is qualified to meet the Code and Specification Requirements. (Ref. CAR 1047-1 Rev. 3, Rev. 1 dated 3/18/81).
- 1. Specimen Removal A. The specimens shall be removed by Duke personnel', under the supervision of a W. J. Woolley representative.
