FIRST INTERIM REPORT Description of Deficiency i
During the performance of a preoperational test en Sequoyah unit 2, it was discovered that the 3/4-inch Masonellan air-operated valves supplied by l
Westin61.cuse which a=e associated with the safety injection system are supplied with a position switch actuator which can rotate about the valve stem.
This rotation results in the less of valve position indication caused by the actuator tissing the valve pcsition switches. This rctatica appears to be a generic condition for this si:e Masonellan valve.
