The following information is submitted in fulfill:ent of the requirements of 3ection 70.22, Title 10, Code of Federal Regulations, Part 70 "Special Nuclear Materials." Two copies are being submitteci.
- 1. Santeich Community High 3chool is a part of Sandwich Community Unit School District Nu=ber 430 in ~,eKalb County in the State of Illinois. The principal office of the school district is located at 720 South Wells Street, Sandwich, Illincis, 60548. The high school is located at 515 East Lions Ro/ai, Sandwich, Illinois, 60548.

- 2. The neutron source is no longer in use. It is kept in storage in the neutron

howitzer described in our application letter dated November 12, 1970, in a comer l
of a locked storeroom between two classrooms in the interior of the high school l
building. The storeroom is kept locked when not in use, and the key to the howitzer j
is stored in the school safe.

- 5. Dr. Kcnneth Ihren is no longar caployed by the Sandwich School District, and has movei to another state. The physica pregran is now under the supervisien of Mr.
Ja:nes M. Liptrap, who received the degree of 3achelor of Science in the Teaching of Chemistry in 1973 from the University of Illinois, and the degree of Master of Education in Secondary Education in 1975 from the same institution. He has no specific training in the handling of radioactive materials, and is therefore not using the regulated source herein described.
- 6. The howitzer used is the same as specified in our ori61nal application. The last l
use of the protection equipment and the last record of exposure was in June of 1978 when Mr. Ihren resi6ned. The materials have been in storage since then.
- 7. Mr. Ihren last performed a leak test on May 24, 1978.
Dr. Villiam J. Adam of your office performed a leak test for us in June,1980, but lef t no written recor:1 thereof. The source was found not to have leaked.
- 8. Any theft or loss of special nuclear material for which this renewc.1 is requested will be reported to the Consission in accordance with section 20.402 of 10 CFR 20 following a telephone call to your office to find out what this section says. Your office will also be notified of our intention to transfer the material either to another educational institution or to a disposal agency so that one of your radiation specialists may sonitor the packa61ng and paperwork involved.
