s TOLEDO EDISON COMPANY DAVIS-BESSE NUCLEAR POWER STATION UNIT ONE SUPPLEMENTAL INFORMATION FOR LER NP-33-80-118 BATE OF EVENT: December 26, 1980 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Failure of the 1-2 Auxiliary Feedwater Pump Turbine outboard bearing Conditions Prior to Occurrence: The unit was in Mode 1, with Power (MWT) = 1580 and Load (Gross MWE) = 530.
Description of Occurrence: On Friday, December 26, 1980, at approximately 1400 hours0.0162 days <br />0.389 hours <br />0.00231 weeks <br />5.327e-4 months <br /> the 1-2 Auxiliary Feedwater Pump was tested per ST 5071.01, Auxiliary Feedwater Monthly Test. It was noted by the designated reviewer that the turbine outboard bearing metal temperature had reached 227.1 F just before the turbine was shutdown but af ter all the data for the test had been taken. The computer continued to print out data after the pump was shutdown until the points were removed from trend. The bearing metal tempera-ture limit is 2200F.
On Tuesday, December 30, 1980, the pump was retested to verify that high temperatures were being reached on the turbine outboard bearing. The bearing reached 2330F during this test. The turbine was shutd9wn and declared inoperable, and the station entered the action statement of Technical Specification 3.7.1.2.
This required the inoperable auxiliary feedwater system to be restored to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in hot shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.
The requirements were met.
The bearing was then removed and found to be wiped. During the disassembly of the bearing it was noted that the oil level was be. low the slinger ring preventing proper lubrication.
Designation of Apparent Cause of Occurrence: The cause of the high bearing temperature was a failed bearing. The bearing failed due to a low oil level. This was caused by a deficiency in procedures ST 5071.01, the Auxiliary Feedwater Monthly Test and SP 1106.06, the Auxiliary Feedwater System. Both procedures called for the oil level to be maintained between the marks on the sightglass. However, the turbine outboard bear-in3 oil level sightglasa haa only one mark. The instructions were inappropriate for the glass installed.
Analysis of Occurrence: There was no danger to the health and safety of the public or to station personnel. The 1-1 Auxiliary Feedwater Pump was operable during the time that the 1-2 Auxiliary Feedwater Pump was inoperable.
Corrective Action
Modification M-4577 was written to ST $071.01, Auxiliary Feedwater Monthly Test and modification M-4578 was written to SP 1106.06, Auxiliary Feedwater System Procedure to specifiy where the oil levels on all the sightglasses should be.
On January 3,1981 the pump was retested and found operable removing the station from the action statement of Technical Specification 3.7.1.2.
