Richland, Washington 99352 Docket No. 50-508 Construction Permit No. CPPR-154 APPENDIX A NOTICE OF VIOLATION As a result of 'he inspection conducted on October 21-24, 1980, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980),
the following violation was iaentified:
10 CFR 50, Appendix B, Criterion XV states that " Measures shall be 6
established to control materials, parts, or components which do not conform to requirements in order to prevent their inadvertent use or installation. These measures shall include, as appropriate, procedures for identification, documentation, segregation, disposition, and notification to affected organizations. Nonconforming items shall j
be reviewed and accepted, rejected, repaired or reworked in accordance with documented procedures."
Paragraph 17.1.15.2 of the Quality Assurance Program states, in part, that "...(c) Site nonconformances dispositioned as repair or use-as-is 4
required approval of the AE engineering as a minimum..."
Contrary to the above, as of August 20, 1980, the Quality Class I ventilation system duct hanger No. 5210 was installed in noncomformance with the architect-engineer's specification No. 3240-232.
One beam clip instead of two beam clips was used to attach the hanger to supporting I-beams. This nonconformance was dispositioned "use-as-is" on Wallace/ Superior Nonconformance Report No. 064 dated May 20, 1980.
This nonconformance disposition had not been approved by -the AE engineering.
This is a Severity Level 4 violation.
