NOTICE OF VIOLATION Northeast Nuclear Energy Company Docket Nos. 50-245 i

50-336 Based on the results of an NRC inspection conducted on July 6 - August 2,1980, 1

it appears that certain of your activides were not conducted in full compliance with the conditions of your NRC License as indicated below. These items are categorized as infractions.

Item A refers to License No. DPR-65; item B and C refer to DPR-21.

A.

Unit 2 Technical Specification 6.8.1 requires in part, " Written procedures shall be established, implemented and maintained covering the...

applicable procedures re.ommended in Appendix "A" of Regulatory Guide 1.33, November 1972"...

Regulatory Guide 1.33 November, 1972 endorses ANSI N18.7 - 1972...

ANSI N18.7 - 1972 section 5.3.5, Maintenance Procedures, subparagraph 3, Post Maintenance Check Out and Return to Service, states in part,

" Instructions shall be included or referenced for returning equipment to its normal operating status... Special attention shall be given to...

systems that can be defeated by leaving valves or breakers mispositioned..."

Contrary to the above, the "A" HPSI pump minimum flow recirculation valve, 2SI-423, was not required by procedure to be restored to its normal operating, locked open position following system maintenance on May 30, 1980. Consequently, during inservice inspection testing on July 3, 1980, damage to the "A" HPSI pump resulted from valve 2SI-423 remaining in the closed position (50-336).

Unit i Technical Specification 6.8.1 requires in part, " Written procedures shall be established, implemented and maintained..."

Health Physics Procedure 4912, Radiation Work Permit Completion and Flow Control, Revision 0, July 1,1980, paragraph 3.1.1 defines Radiation Work Pennit (RWP) areas, in part, as "... any areas posted with an "RWP Required" sign,..."

All work in the Unit 1 Reactor Building overhead is posted as "RWP Required".

Contrary to the above, on July 23, 1980, four contractor employees were erecting portable staging in the Unit 1 Reactor Building overhead without a RWP. (DN 50-245) i rlon5cS4G(

10 CFR 19.12, Instructions to Workers, states in part, "All individuals working in or frequenting any portion of a restricted area shall be kept infonned of the storage, transfer or use of radioactive materials or of radiation in such portions of the restricted area..."

Contrary to the above, on July 23, 1980, four individuals were not in-fonned of the presence of loose surface contamination and other radiolo-gical hazards, such as the potential to generate airborne radioactive material prior to working in the reactor building overhead.

Subsequent surveys showed up to 220,000 dpm/100 cmz removable contamination. The individuals, who were not wearing protective clothing, were found to be 2 beta-gama. (DN 50-245) contaminated to levels of 180,000 dpm/100 cm l
