l Condition 1 -1.icensee shcIl file with the Customs Officer or the Postmaster two copies,in addi-tion to those otherwise required, of the Shipper's Export Declarction covering each export and merk one of such copies for trcnsmittoi to the U.S. Nuclear Regu-Ictory Commission, Washington, D.C. 20555. The following declerction should oc-company or be picced on the Shipper's Export Declcrations for such exports:
"This shipment is being made pursuant to specific license number (specific license number) filed et (location of Customs office where license is filed),
on (date license was filed). This license expires on (expiration date of license), and the unshipped balance remaining on this license is sufficient to cover the shipment described on this dec!cretien.'
Condition 2 - Exports authorized in any country er destinction, except Country Groups Q, S, W, X, Y, and Z in Pcrt 370, Supplement No.1, of the Comprehensrve Export Schedule of the U.S. Depcrtment of Commerce.
Condition 3 - This license covers only the nuclecr content of the meterici.
Condition 4 - The materici to be exported under this license shcIl be shipped in accordance with the physical protection recuirements for special nuclecr meterici in 10 CFR 73.
Condition 5 - Special nuclecr meterici cuthorized for excert under th s license shcil not be transported outside the United Stctes in passenger carrying circraft in shipments exceeding (1) 20 grams or 20 curies, whichever is less, of plutonium or uranium 233, or (2) 350 grcms of uranium 235.
Condition 6-Thir. license authertzes export on:y cnd does not cuthori:e the rece;pt, physrcol possessien, or use of the nuclear materici.
Condition 7 - The licensee shcIl complete end submit en NRC Form 741 for each shipment of source materici exported under this license.
Condition 8 - The licensee shall advise the NRC in the event there is any change in the designo-tien of tne company who will pockcge the nuc!ecr material to be exported under this license, or any chcnge in the lecction of the pcckaging operation, et least three weeks prior to the scheduled date of export.
