Dear Mr. Keppler:

On June 19, 1930, Mr. T. D. Geib of Public Service Company of Indiana, Inc., (PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55 (e). The same item was also reported by Westinghouse under 10 CFR 21 (see Westinghouse letter no. NS-TMA-2264, dated June 18, 1980, to the Director, Office of Inspection and Enforcement).

Westinghouse identified intermittent contact operation as a problem occuring in their Type W-2 switch when it is set in the neutral position.

j This switch is used in a number of safeguards systems and this problem can defeat the automatic actuation function of the switch.

Westinghouse has been studying intermittent contact operation of their Type W-2 switches and presently continues to recommend the interim solution of rewiring indicator lights as described in Westinghouse NSD Technical Bulletin No. NSD-TB-80-9.

PSI will implement this interim solution if a permanent solution is not available six months prior to fuel load. PSI will continue to monitor the efforts of Westinghouse in this matter and implement the necessary changes.

This letter is intended to fulfill the requirements of a final report as defined in 10 CFR 50.55(e).
