Gentlemen:
This letter refers to the inspection of your activities authorized
under AEC Facility License Nos. DPR-2, DPR-19, and DPR-25, conducted
'sy Mess. s. Dance, Johnson and Knop of this office on June 10-14, 1974,
at the Dresden Nuclear Station near Morris, Illinois, and on June 25-
26, 1974, at the Occmonwealth Edison Company offices in Chicago,
A copy of our report of this inspection is enclosed and identifies the
areas examined during the inspection. Within these areas, the inspec-

tion consisted of a selective examination of procedures and represen-
tative records, interviews with plant personnel, and observations by
the inspectors.

During this inspection, it was found that certain of your activities
appeared to be in violation of AEC requirements.
These matters, which
Messrs. Fiorelli, Knop and Keppler discussed with Messrs. Behnke and Lee
on July 30, 1974, at the Commonwealth Edison Company offices and which
Messrs. Dance and Knop discussed with other members of your Senior
Staff, at the Dresden site on July 26, 1974, are identified under
Enforcement Action in the Susnary of Findings Section of the enclosed
inspection report.
This notice is sent to you pursuant to the provisions of Section 2.201
of the AEC's " Rules of Practice," Part 2, Title 10, Code of Federal
Regulations. Section 2.201 requires you to submit to this office
within cwecty days of your receipt of this notice, a written statement
or explanation in reply, including: (1) corrective steps which have
been taken by you, and the results achieved; (2) corrective steps
which will be taken to avoid further violations; and (3) the date when
full compliance will be achieved.
Since our meeting with you on February 14, 1974, we have observed
measurable improvements in the operation of the Dresden Station;
-
however, the actions taken have still not led to the prevention of
similar problems as evidenced by the findings from this inspection.

While the progress observed during the past few months is encouraging,
it is F.portant that management continue its efforts,to assure that
all Drenden plant personnel recognize that safe operations and full
compliance with AEC related commitments are top management priority
Furthermore, based on our inspection findings to date, na
matters.
are unable to recommend at this time the issuance of the full term
Operating License'for Dresden 2.
As discussed with Messrs. Behnke
and Lee by Messrs. Fiore111 and Keppler on July 30, 1974, a favorable
recommendation by the Directorate of Regulatory Operations in support
of the Dresden 2 full term license will be contingent on the correc-
tion of the violations identified during this inspection, the steps
taken to prevent the recurrence of these and similar violations, and
the implementation of effective management controls to assure the
safety of operations and full compliance with AEC requirements.
The major fundamental areas chat need your additional attention
include:
1.
Marginal review and followup by members of the technical staff.
~
These, we believe, have resulted from inexperienced personnel and
high staff turnover.
2.
Overtaxing of supervisory personnel. We believe this has resulted
in a reduction in the effective supervision of operating, mainte-
i
nance and QA/QC activities due to less than complete - pervisory
]
planning and followup.
~
3.
Lack of organized training in quality control methods and lack
of complete implementation of your established quality control
procedures. These we believe, have resulted in incomplete under-
standing of quality and administrative requirements and consequently
have adversely affected the implementation of established controls
by those responsible for the conduct of these activities.
4.
Lack of coordinated and updated administrative procedures. This
is resulting in a lack of clarity of applicable procedures which
leads to an increased potential for problems due to questions
relative to the applicability of procedures.
In your reply to this letter, please provide us with your comments on
the actions taken or planned to increase your effectiveness in those
areas.
