O Robertshaw Controls Co. - Fulton Syphon Division 99900722/80-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on June 23-24, 1980, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR 50 and paragraph NCA-4134.5 of Section III to the ASME Code states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, _ or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or dr: wings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Deviations from these requirements are as follows:
Paragraph 5.4.3 of Section 5 of the ASME accepted Ouality Assurance Manual states, "When the check list is revised the revision designation shall be changed and the revision reviewed and appreved by the individual who per-formed the ;nitial approval...."
Contrary to these requirements, a Master Checklist, had been revised, using
" white out," to conform with the data on Process and Cost Cards, for new orders without changing the revision designation, and without having the revision reviewed and approved by the individual who perfonned the initial approval.
