Dear Mr. Lewis:

T. ' s letter refers to the apparent deficiency in the sub-ject inspection report which states:

i As required by 10 CFR 50, Appendix B, Criterion V, "Activi-ties affecting quality shall be prescribed by documented

... procedures...and shall be accomplished in accordance with these... procedures... ".

The accepted Quality Assur-ance Program, Chapter 11, Paragraph 11.4.1 states in part "The test program is to be identified, documented, sche-duled, and accomplished in accordance with controlled pro-3 cedures".

Startup Standard 6, Paragraph 5.6.4 states in part, l

"The Official Test copy of the procedure, including l

all approved modifications, will be followed in per-forming the test."

Test Procedure 064-5-017, RCS Leak Rate Preoperational Test Paragraph 8.1 requires an inspection for system leakage and elimination or minimization of such leakage prior to conducting the test.

Contrary to the above, during the performance of test procedure 064-5-017 an inspection for system leakage and elimination or minimization of such leaks was not completed prior to the official leak test.

Startup Standard 6, Paragraph 5.6.6 states in part

...When it becumes necessary to revise a data entry, due to an entry error or any other reason, the entry originally entered will be struck through and the correct or revised data entered above, below, or j

beside the original entry.

The revision will be initialed and dated by the person making the re-l vision."

Contrary to the above, during the performance of Test Procedure 064-4-015 RTO/ Thermocouple Cross Calibration Preoperational Test, corrections to data in data sheets 3, 6,

7, 8,

12, 13, 14, and 15 were not properly lined through, initialed, and dated.

Response

The following corrective action has been accomplished:

An investigation was conducted r".

" inspection for system leakage and elimination or minimization of such leakage" which revealed that (1) the test supervisor was under the impression that this step was to be signed off after the conduct of the test to ensure elimination or minimization of all iden-1 tified leakage and (2) the test supervisor had i

observed and taken steps to minimize identified leakage for approximately five days prior to con-4 duct of the test.

A Test Data Deficiency Sheet (TDDS) was written to reflect the above and has been attached to the sub-ject test procedure.

On July 15, 1980 a training session was i. eld with the Startup Engineers with respect to the approved method of conducting tests and recording data.

We do not consider the information contained in the subject report to be of a proprietary nature.
