Technical Specifications, Paragraph D.1 states, in part,
"...All plant operations...shall be conducted in accordance with written procedures Yankee Operating Procedure, Primary Systen "ater Balance, OP-4220, a.

Contrary to the above the calculations performed on !! arch 2 and 3, 1974, "a o not acco:plished in accordance uith the procedures.

Yankee Operating Procedure, OP8100, Establishing and Posting Con-trolled Areas, states, in part,
---All contrclied areas shall be adequately surveyed to insure that the boundaries established con-pletely define the controlled area--."
Contrary to the above, on >brch 1.2, 1974, the boundary established did not completely define the contaminated area identified in the New Fuel Storage Vault.
