South Texas Project Units 1&2 Docket Nos. STN 50-498, STN 50-499Y Final Report Concerning the Breakdown in the Vendor Surveillance Program for Class 1E Electrical Equipment On September 18, 1980, Houston Lighting & Power Company notified your office of a potentially reportable deficiency concerning a possible breakdown in the Vendor Surveillance Program for Class 1E Electrical Eciuipment. Upon review, HL&P has detemined that the safety-related equipment purchase orders that were identified in the deficiency reported on September 18, 1980 are a sub-part of the generic deficiency that was reported to your office on June 13, 1980, as a " Breakdown in the Procurement Cycle of Purchased Materials,"

and should be tracked in a similar manner. This position has been discussed with the South Texas Project NRC Resident Inspector. Therefore, HL&P con-siders this particular item to be closed as a separate issue, but will in-corporate this item as part of the generic deficiency that was reported on June 13, 1980.

Please note that this deficiency has been reported by Brown & Root, Inc. .

Pursuant to the discussion on October 3, 1980 between your Mr. Westerman (Region IV Duty Officer) and Messrs. A. H.

Geisler and P. S. Jordan of Brown 6 Root, Inc., thisinletter provides written notification of a reportable defect accordance with 10CFR21, regarding Power Conversions Prod-ucts of Crystal Lake, Illinois (hereafter referred to as the

" supplier").

Sixteen (1_6) Class IE battery charges have been provided to the South Texas Project Electric Generating Station (STPEGS) by the supplier under purchase order 35-1197-4100/8100.

spection performed at STPEGS by Brown 6 Root Quality Engi-neering, several deficient conditions were detected with one battery charger as documented in Brown 6 Root nonconformance report (NCR) S-E2832. A more detailed inspection of all sixteen (16) battery chargers was conducted by Brown 6 Root Electrical Design Engineering to fully define the equipment defects. Deficient conditions encountered include the following:

1.- Various electronic components were provided by foreign suppliers which is in violation of specification requirements.

2. 'High voltage terminals located too close to cabinet frane and taped to attempt to achieve separation.
3. Power lugs have improperly sized holes.

4. Split lugs from over crimping on DC output control wiring.
5. Use of wire, lugs, and fuses which deviate from the specification and have not been approved by Brown & Root. In' some cases, unapproved components are also not in accordance with the accompanying bill of materials.
6. Metal filings on transformer which could cause vibration induced insulation degra-dation.
7. Poor workmanship in general, e.g. metal filings, solder splatter, nicked and cut wire insulation, and improperly installed pin connectors.

The above deficient conditions taken individually or col-lectively could, in our judgment, compromise the Class  :

IE qualification of the battery chargers. The results of the detailed Engineering inspection was discussed at the B&R Incident Review Committee on October 3, 1980 and the decision was to report the defects to the NRC in accordance with the requirements of 10CFR21. Brown & Root Corrective Action Report S-332 places a " Hold" on the sixteen (16) battery chargers until corrective action has been completed.

These defects are, however, considered repairable in the field. Specific corrective action will be developed jointly among Brown & Root, the supplier, and the licensee.

Relative to the provisions and intent of 10CFR21, we believe the deficient equipment from Power Conversion Products, as described herein, may be of significance in a generic sense.

Power Conversion Products is an industry supplier of Class IE electrical equipment. An undetermined amount of such equipment with similar defects may have been provided to other licensees.
