AFC0 Steel Docket No. 99900718/80-01 NOTICE OF DEVIATION Based on the rt.ults of an NRC inspection conducted on July 10 and 11, 1980, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Deviations from these requirements are as follows:
The customer specification requires all full penetration tee welds to be tested as follows: Magnetic particle or liquid penetrant inspection of root pass and final weld layer and ultrasonic test completed welds using the angle beam technique.
Contrary to the above, the full penetration tee welds joining the 2 inch anchor bolts to the base plate of the embedments were not tested using the angle beam technique.
Acticas to correct the deficiency and to prevent recurrence has been completed, therefore no further response to this item is necessary (Details Section, paragraph C.4.b).
