Appendix A NOTICE OF VIOLATION Illinois Power Company Docket No. 50-461 Based on the inspection conducted on August 5-8, 1980, it appears that certain of your activities wt-e in noncompliance with NRC requirements, as noted below. This item is an infraction.
10 CFR 50, Appendix B, Criterion V requires in part that " Activities affecting quality shall be prescribed by documented instructions, pro-cedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or dr: wings."
The Illinois Power Company PSAR, Section 17.1.5.1 states in part that,
" Requirements for the use of instructions, procedures, sketches, drawings, and related material to control activities which may affect quality are generated and implemented within using organization. Formal procedures, instructions, and drawings shall be developed and used as appropriate, 2
for the activities affecting quality".
Contrary to the above, the licensee failed to follow procedures and instructions in the implementation of the welding requirements of the control rod drive housing to the reactor vessel botton head penetration, in that, Reactor Controls Incorporated (RCI), Process Requirements Sheet for Reactor Pressure Vessel Internals Installation Work, Weld No. 32, required a mock-up weld using WPS 34/34-0TS-1 IG/2F position. The Performance Qualification Records of mock-up No. QNU-32 indicated that six of the nine welders performed the mock-up using the 1G/1F position.
