Gentlemen:
This refers to the inspection conducted by Messrs. W. D. Shafer,
F. A. Maura, and I. N. Jackiv of this office on September 8, 9,
12-16, end 21,1977, of activities at the Dresden Nuclear Power
Station, Units 1, 2 and 3, authorized by Licenses No. DPR-2,
No. DrR-19 and No. DPR-25 and to the discussion of our findings
,
with Mr. Stephenson at the conclusion of the inspection.
The enclosed copy of our inspeccion report identifies areas
examined during the inspection. Within these areas, the
inspection consisted of a selective examination of procedures
and representative records, observations, and interviews with
personnel.
During this inspection, certain of your activities appeared
to be in noncompliance with NRC requirements, as described
in the enclosed Appe.ndix A.
This notice is sent to you pursuant to the provisions of
Section 2.201 of the NRC's " Pules of Practice," Part 2, Title
10, Code of Federal Regulations. Section 2.201 requires you
to submit to this office within twenty days of your receipt
of this notice a written statement or explanation in reply,
including for each item of noncompliance: (1) corrective
action taken and the results achieved; (2) corrective action
to be taken to avoid further noncompliance; and (3) the date
when full compliance will be achieved.

Unit 3 APRM system. These commitments are identified in paragraph
10 of this report.
On. November 14, 1976, you reported to this office the failure of
- the HPCI injection valve stem on Unit 2 HPCI system. Since that
.
'
time that valve has been left open and out of service except for
periodic valve operability and surveillance votk. On August 2,
'
1977, this ease valve fmiled to open upon completion of your rou-
tine surveillance work. The problem with the valve at the time
j
appeared to have been corrected and the valve was once again locked
in the open position. However, on September 10, 1977, when the valve
was again used for HPCI surveillance, the valve failed to open as
'
required after being closed. Preliminary information once again
indicates that the valve stem has beten damaged during the attempts
-
to operate the valve. As a result of these reoccurring problems on
the Unit 2 HPCI valvo, you are requested to provide this office with
a report, within 70 days of receipt of this letter, on the following:
'
1.
Your conclusions regarding the cause of the failures on the
HPCI system valve 2301-8.

2.
Your conclusions regarding the degraded atmospheric conditions

in the Unit 2 X-area, where this valve is located.
3.
The actions taken or planned, to eliminate the HPCI valve
.
failure that has occurred in the past.

With respect to the noncompliance identified in Appendix A of this
!
report, relating to the failure of the secondary containment per-
sonnel access doors, we request that you provide us within 20
days of receipt of this report, your plans to implement the modi-
fication'to correct the door problems that was initiated by you
on March 10, 1975. We also request that you inform us of the
interim actions to be taken to assure proper closure of these sec-
,
ondary containment personnel access doors on both units.

In accordance with Section 2.790 of the NRC's " Rules of
Practice," Part 2 Title 10, Code of Federal Regulations, a
copy of this letter, the enclosures, and your response to
,
this letter will be placed in the NRC's Public Document Room,
except as follows. If the enclosures contain information
that you or your contractors believe to be proprietary, you
must apply in writing to this office, within twenty days of
