APPENDIX A NOTICE OF VIOLATION Tennessee Valley Authority License Nos. CPPR-172 Yellow Creek Nuclear Plant
& CPPR-173 Based on the NRC inspection July 15-17, 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
As required by Criterion V of Appendix B to 10 CFR 50, and as implemented by TVA Topical Report TVA-TR75-1A, Section 17.1 A.5, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings...and shall be accomplished in accordance with these instructions, procedures, or drawings". As stated in ASTM C31 and implemented by Quality Control Inc.ruction QCI C-208,
... test specimens shall have free water maintained on the entire surface area at all times."
Contrary to the above, on July 16, 1980, the inspector observed test cylinders 2021, 2022, 2023, 2024, 2157, 2158, and 2173 for safety-related concrete pours were partially dry.
This is an infraction.
