Conditions Prior to Occurrence: 59% FP  ;

Description of Occurrence:

At 0700 on August 22, 1980, the Unit 2 Concentrated Boric Acid Storage Tank (CBAST) pump was removed from service. An oil seal, flange gasket, worm gear set, and pressure relief valve were replaced. The rump was returned to service l

and declared operable at 0115 on August 24, 1980. This constitutes operation in a degraded mode per Technical Specification 3.2.2 and is thus reportable pursuant to Technical Specification 6.6.2.1.b(2). )

Apparent Cause of Occurrence:

maring the cleanup of the CBAST room, an oil leak and a flange leak were discovered on the CBAST pump. Work requests were written for inspection and repair. During the inspection, the oil seal was found to have been torn and the worm gear showed some wear.

I was installed. The flange leak The oil ~ seal was replaced, and a new worm gear set was repaired by installing a new gasket. The pump would not pump water when it was tested for operability. The pressure t

relief valve was found to be stuck, and it was replaced and set to the desired pressure.

A parformance test was run and the pump was declared operable at 0100

on August 24, 1980.

I Analysis of Occurrence:

The ability to achieve the necessary shutdown margin in the event of an accidert is dependent on the ability of the unit to borate the primary system. The primary method- for accomplishing this is the use of the CBAST Pump. When the CBAST Pump is inoperable, the system can be borated by using the Borated Water Storage Tank (BWST) to feed the high pressure injection pumps directly. The  !

BWST was available while the CBAST pump was inoperable, thus insuring a means 1 of safely shutting the unit down. Furthermore, the pump was restored to l operability Thus, well within the '72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> permitted by Technical Specification 3.2.2.

this incident is considered to be of no significance with respect to safe ,

operation, and the health and safety of the public were not affected.

Corrective Action:

After the leaks were discovered; a new oil seal was installed to replace the torn one; the worm gear set was found to have some wear and was replaced as a preventive ceasure; the gasket was replaced on the leaking flange; and the Physical Maintenance procedure will be changed to reflect oil seal and flange inspections. A station modification will be initiated to add an additional pump to the system and/or replace the existing pump with one of a different type.
