Narrative

Report for LER 50-366/1980-136 i

During routine startup activities, the site was notified by the AE (Bechtel) of a

PSW division I

line that was inadequately supported when subjected to OBE or DBE loads.

l This deficiency revealed piping seismic stresses that exceed l

code allowables and,was, discovered during a

review of l

"as-built" piping data acquired in response to IEB 79-14.

The cause of this high stress condition was due to the f ailure-to install a

3-directional support on the piping.

Assuming catastrophic failure of the subject piping, the cooling water.

(PSW div. I) would be severely compromised thereby renderng

the associated components (area coolers for RCIC,

RHR, CA, L'

jockey pumps and. DWL chillers) inoperable during a -seismic event.

Due to this ' fact, Rx startup activities were suspended, and the unit was _ considered in a

12-hour LCO per Tech Specs section

3. 5.

Upon notification by the AE efforts were begun-immediately to implement the design change necessary to resolve the condition.

This change was satisfactorily implemented within 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

Similar-PSW piping (div. II) was available-and operable to equipment redundant to the l

inoperable equipment.
