SUBJECT: NRC INSPECTION OF THE TROJAN NUCLEAR PLANT

This letter refers to the routine inspection by Messrs. J. Burdoin and F. Gee
of this office on October 16-20, 1989, and additional document review in
i
the regional _ office during the week ending October 28, 1989, of activities
authorized by NRC License No. NPF-1. Findings were discussed with members of
'
your staff at the' conclusion of the inspection.
!

Areas examined during this inspection are described in the enclosed inspection
!
report. Within these areas, the inspection consisted of selective
examinations of procedures and representative records, interviews with
,
personnel, and observations by the inspectors.
!

Based on the results of this inspection, it appears that two of your
.
activities were not conducted in full compliance with NRC requirements, as
.
set forth in the Notice of Violation, enclosed herewith as Appendix A.
!
This inspection identified a number of deficiencies.
In porticular, the
!
'following problems were noted in conjunction with review of engineering
calculations and emergency procedures associated with a loss of power to
the Service Water System during a fire:

(1)- An important calculation assumption was based on inaccurate data.
(2) An independent licensee review of the calculation had raised questions
,

about the adequacy of the applicable emergency response procedure
which were not properly addressed.

(3) The applicable emergency response procedure was found to be deficient
'
in that it did not provide sufficient detail to enable plant operators
to perform the required actions.
,
The above noted deficiencies are indicative of weaknesses in the quality of
I
.
technical work being performed by PGE.
You should ensure that these
-
deficiencies are not programmatic in nature and take appropriate steps to
avoid similar deficiencies in other procedures and calculations.
