OnLSeptember 18 - 22, 1989, the NRC administered requalification examinations

- to employees of your company who operate the Millstone Nuclear Power Station,

. Unit No. 3. .The' evaluation was conducted in-accordance with NUREG-1021, Opera-

As evaluated =by the NRC, six of twelve operators failed one or more segments-of

the examination, and one crew failed the simulator portion of the examination.

As a result of this evaluation, your requalification program has been assigned-

an overal1 }rogram rating of " unsatisfactory." The evaluation was conducted in

accordance with the criteria of NUREG-1021, ES-601, noted above. The six'

-Following the completion of the examination, a meeting was held on September

"

25, 1989, at the NRC Region 1 office in King of Prussia, Pennsylvania to

-

discuss the results of the evaluation and various courses of-actions resulting

from the unsatisfactory program evaluation.

Results of this meeting are

.

detailed in the enclosed report.

- During the September 25, 1989 meeting in Region 1, Northeast Nuclea'r Energy

Company (NNECO) committed.to provide a letter to the NRC identifying additional-

actions =taken or planned to justify continued safe-operation of Millstone Unit

'

On September 28 - 29, 1989, the NRC conducted performance assessments of two

other operating crews on the simulator to verify that the interim measures

implemented by the licensee would assure continued safe operation of Millstone,

Unit 3.

As a result of these assessments, the interim measures were found to

be satisfactory for continued operation as detailed in the enclosed report.

In a letter dated October 3, 1989, NNECO identified long term improvements to

be made in the requalification training program and rules of usage applying to=

Emergency Operating Procedures. This letter satisfied the NNECO commitment

made at the September 25,-1989 meeting.

In summary, the interim measures relative to continued safe operation and the

long term improvements identified by NNECO are considered acceptable; however,

the Millstone 3 requalification program rating will continue in an unsatisfac-

tory status until such time. as an NRC administered requalification exam receives

an.overall program rating of satisfactory.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this

letter and the enclosure will be placed in the NRC Public Document Room.
