NOTICE OF VIOLATION Portland General Electric Company Docket No. 50-344 TrojanNuclearPlant License No. NPF-1 During an NRC inspection conducted from September 9 through October 13, 1989,

-violations of NRC requirements were identified.
In accordance with the
" General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1989), the violations are listed below:
A.
10 CFR Part 50, Appendix B, Criterion XII states that "Heasures shall be establishedtoassurethattools, gages, Instruments and other measuring andtestingdevicesusedinactivitiesaffectingqualityareproperly controlled calibrated, and adjusted at specified periods to maintain accuracywIthinnecessarylimits."
Contrary to the above, dial indicators used to determine dimensions on safety-related ecuipment were not in the licensee's program for calibrationoracjustmentetspecifiedperiods.
Specifically, such dial indicators were used under Maintenance Request (MR) 89-9141 to inspect the thrust bearing clearances for the Centrifugal Charging Pump (CCP) shaft.
This is a Severity Level IV violation.
B.
10 CFR Part 50 Appendix B, Criterion V, states that " Activities affectingqualltyshallbeprescribedbydocumentedinstructions, procedures,'or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily
' accomplished."
MaintenanceProcedure(MP)2-0, Revision 12,"InstalledPlantInstrument Maintenance and Calibration," describes the general guidelines for maintenance and calibration of installed plant instrumentation.
MP 2-0, states that "If the As Found readings.for quality-related instruments are outside the stated tolerance (or if the instrument is

completely failed), the I&C technician shall complete the testing portion of the procedure, where applicable and bring it to the attention of the I&C Supervisor or his designee.
They shall initiate a Form I&C-10, Installed Instrument Out-of-Calibration Investigation for Quality-Related Instruments."

recorded the as-found value as 791.5 volts for the detector voltage, L
'which was outside the 800 15 volts criterion, but the licensee did not

initiate an out-of-calibration investigation.
This is a Severity Level IV violation.

Document Control Desk, Washington,DC20555wkthacopytotheRegionalAdministrator,RegionV, and a copy to the NRC Senior Resident Inspector Tr within 30 days ofthedateofthelettertransmittingthisNotIce.ojanThIsreplyshouldbe clearly marked as a " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation if admitted,- (2) the corrective steps that have been taken and the results achieved, (3)d (4) the corrective steps that will be taken to avoid further violations, an the date when full compliance will be achieved. -If an adequate reply is not received within the time specified in this Notice, an order may be i
issued to show cause why the license should not be modified, suspended, i
or revoked or why such other action as may be proper should not be taken.
Consideration may be given to extending the response time for good cause shown.
