NOTICE OF VIOLATION Louisiana Power & Light Company (LP&L)

Docket: 50-382 Waterford Steam Electric Station, Unit 3 Operating License: NPF-38 i

During an NRC special inspection conducted on September 21-29, 1989, of your t

activities associated with the testing of a main steam line code safety valve, two violations of NRC requirements were identified.

In accordance with the Part 2. Appendix C (1989) (y and Procedure for NRC Enforcement Actions," 10 C

" General Statement of PolicEnforcement Policy), the violations are listed below:

A.

Inoperable Main Steam Line Code Safety Valve Technical Specification 3.7.1.1.a requires, in part, that with both reactor coolant loops and associated steam generators in operation and with one or more main steam line code safety valves inoperable, operation in MODES 1, 2, and 3 is prohibited, unless, within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br />, either the inoperable valve is restored to OPERABLE status or the linear power level-high trip setpoint is reduced per Table 3.7-2 of the Technical Specifications.

Contrary to the above, on September 21, 1989 With the unit operating in Mode 1 Main Steam Line Code Safety Valve MS-106A was determined to be inoperable based on confirmation of test data taken at 12:41 p.m. (CDT).

1he valve was not returned to OPERABLE status and the linear power level-high trip setpoint was not reduced per Table 3.7-2 within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br />.

This is a Severity Level IV violation.

Failure to Provide an Adequate Main Steam Line Code Safety Valve Testing Procedure Technical Specification 6.8.1.c requires that written procedures shall be established, implemented, and maintained for surveillance and test activities of safety-related equipment.

Procedore HM-007-015 Revision 0, l

"Trevitest of Main Steam Safety Valves," provides for testing of safety-related main steam line code safety valves.

Contrary to the above, it was found during the period September 21-29, 1989, that Procedure MM-007-015 was inadequately established in that the licensee had not determined if the installation of the Trevitest test unit affected the operability of MS-106A. Eight other deficiencies documented in NRC Inspection Report 50-382/89-31 were also noted with Procedure MM-007-015.

This is a Severity Level IV violation.
