1 NARRATIVE REPORT UNIT 1 August 2nd At 2030 reduced load in order to perform a rod pattern adjustuent August 4th At 17:0'O Rx auto scram on loss of EHC DC input power.
Loss of DC power was due to a blows 3-amp fuse in EHC cabinet.
Fuse blev because of overload caused by improper indicating light being placed in control panel August 5th Startup from above scram.
Rx critical at 04:33 and generator on line at 11:44 August 5th Reduction due to increased condenser vacuum August 6th At 18:10 reduced load to swap SJAEs August 9th Reduced load in order to perform a rod pattern adjustment August 12th At 23:03 Rx auto scram on low wt.'er level because of loss of feedpump.
Generator off line at 22:57 August 13th Startup from above scram.
Rx critical at 08:36 and generator on line at 16:01 August 16th At 20:30 reduced load to perform a rod pattern adjustment August 18Ih Reduced load in order to properly align 4-way valve in PSW to H2 cooler.
Valve was not properly aligned to allow total PSW flow to pass through H2 cooler August 21st At 2000 reduced load because of loss of vacuum because valve N62-F003B had stuck in closed position August 22nd At 2000 reduced load because of loss of PSW.
Loss PSW because of sheared keyway in Division II strainers.
Keyway sheared due to debris accumulation on strainer There was no single release of radioactivity or single radiation exposure which accounts for more than 10% of the allowable annual values during August of 1980.

Mater was hanging up.
80-4191 8-12-80 Replaced motor on RER inboard injection valve.
Motor was found to be burnt up. Ref. DCR 80-274 80-3897 7-29-80 Replaced breaker on RHR d/w spray MOV.
Breaker would not lock in closed position 80-3700 8-11-80 Relay C71-81-3F was found to be chattering.
This chattering w s.s caused by a faulty under frequen-cy monitor.
Replaced and calib-rated under frequency relay 80-3905 7-28-80 Modified anchors on E51-RCICH-23 to provide adequate seismic support per DCR 80-259 80-3081 8-4-80 Replaced CRD HCU pressure switch isolation valve.
Valve was leaking when switch was isolated.
80-35b4 8-5-80 Installed limiter in HPCI controller per DCR 80-218 80-3948 8-1-80 Inspected and repaired woodward governor drive gears and speed pickup gears on HPCI 80-4130 8-22-80 Cleaned top isolation valve stem treads for Ell-N040.
