Facility: Trojan Nuclear Plant, PO Box 439, Rainier, Oregon 97048 4.
Identification of Occurrence:
The fan for the west containment spray pump room was inoperable as a result of a concrete core drilling operation.

Conditions Prior to Occurrence:
The plant was in all modes of operation including 100% of rated power while the fan was inoperable.

Description of Occurrence:
One-inch conduit AB-4596 supplying power to the MV252A fan for the west containment spray pump room was damaged by a concrete core drilling operation. The conduit was attached to the underside of a floor that was being drilled through.
The conduit was not embedded.
The conduit was damaged someitme during May 1980, but due to various personnel errors it was not known until August 3, 1980 that a safety-related piece of auxiliary equipment was inoperable.

Designation of Apparent Cause of Occurrence:
The cause of this occurrence was personnel error.
The core drilling crew did not take adequate precartions prior to commencing the core drilling operations.
8.
Analvsis of Occurrence:
This occurrence had no effect on plant or public safety.
The redundant containment spray pump and associated fan was fully operable and avail-able for use.
9.
Corrective Action:
The permanent corrective action taken was to change the procedure under which core-drilling is done onsite. From now on, voice communication
~ (either radio or telephone) must be established between the core drilling crew on one side of the wall (or floor, etc.) 'and an observer on the other side of the wall.
This will ensure that the core drill does not encounter any attached plant equipment,' conduit, etc., and if that does happen, it can quick 1v be stopped.
