STN 50-482/80d 'i Appendix A NOTTCE OF VIOLATION Based on the results of the NRC inspection conducted on June 6-10, 1980, it appears that certain of your activities were not conducted in full compliance with the conditions of your NRC Construction Permit No. CPPR-147 as indicated below:
Failure to Establish Housekeeping Zone Designations 10 CFR 50, Appendix B, Criterion V states, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
The Preliminary Safety Analysis Report (PSAR), Section 3.12, " Regulatory Guides," states, "The standards as set forth in ANSI N45.2.3, ' Housekeeping During the Construction Phase of Nuclear Power Plants,' will be followed."
ANSI N45.2.3, Section 2.1, " Planning," requires that cleanliness require-ments be established on the basis of specific zone designations, and that the time for the implementation of zone designations shall be as required by construction progress.
Daniel Construction Procedure AP-XIII-05, " Project Housekeeping," paragraph 4.1.3 requires-the construction manager to review construction schedules and available information and to assign housekeeping zone designations (Zone I, II or III) in compliance with ANSI N45.2.3.
Contrary to the above:
On June 6, 1980, a specific zone designation (Zone I, II or III),.in compliance with ANSI N45.2.3 and associated cleanliness requirements commensurate with the current construction activities, had not been assigned or implemented by the construction manager for the reactor refueling cavity in the Containment Building.
The refueling cavity was open for work on the installed reactor vessel during the morning of June 6,1980. The reactor internals are also stored in the reactor refueling cavity.
This is an infraction.
