Dear Mr. Keppler:

On July 24, 1979, Mr. C. E. Chmielewski of Public Service Company of Indiana, Inc. , (PSI) telephoned your office to notify you of a potentially reportable incident as required by 10 CFR 50.55(e).

This incident was the failure of Newberg-Marble Hill (N-MH) to incorporate Contractor Change Request (CCR) 200 into its concrete pour located at the 383' elevation in the Auxiliary Building. The pour was made on March 29,1979, using 3500 psi concrete instead of the 5500 psi concrete requir&d by CCR 200.

l As discussed in the interim report of August 23, 1979, Sargent

& Lundy Engineers performed an evaluation of the pour area and I

found that, based on the 91-day compression data, the concrete strength was adequate to meet design requirements. Also as discussed in the interim report, PSI investigated the potential breakdown of

! the N-MH Quality Assurance Program. It was determined that the delayed l

reporting of the incident by N-MH QA personnel was an isolated occurence, however some deficiencies did exist in the N-MH QA Program. N-MH personnel could previously initiate Contractor Change Requests without notifying their QA personnel and hang hold tags on pours without logging or notifying QA personnel until after the Contractor Change Request was approved. These deficiencies have been corrected and N-MH QA personnel must now be immediately notified of Contractor Change Request initiation and hold tags will be hung by N-MH QA personnel.

N-MH has also conducted training for timely reporting of deficiencies.
