. Report Date: ~ August 7, 1980 Facility: McGuire Nuclear Station, Units 1 and 2 Identification of Deficiency: Defect.ive 6011 Leads on Valcor Solenoid Valves Description of Deficiency:
On July 7, 1980, Mr. T. F. Wyke, Mr.' J, R. Wells, and Mr. C. P. Tegard reported the following deficiency, by telephone, to Mr. Milt Hunt of NRC, Region II, Atlanta. The deficiency was origirially reported by Stone and Webster in two Valcor solenoid valves Model V70900-11-3. An examination was conducted of these valves by Valcor. It was determined that the first valve coil failure was caused by a magnet wire at a wire splice protruding through or near the outer surface causing a short to the solenoid housing. This resulted in a hole being burned through the outer molded material. A study of the second valve revealad that the start wire lead joint was touching the. windings at one spot. Insulation at this point was cut too short to provide complete insulation between the lead wire joint and the winding. Although the coil was visibly undamaged, coil resistance was nil.
Components as identified as having this deficiency are Valcor solenoid valve coils, Model V70900-21-3, S/N 738A to 245A and 746 to 807.
Analysis of Safer! I,mplication:
Short circuiting of the solenoid valve coils would cause excessive curt.ent and eventual tripping of circuit breakers. This would de-energize the solenoid valve and result in closure to the safe position. However, the short would also disable all other controls on the same circuit. Ihis constitutes a significant deviation from performance specifications of the valves and circuits.
Corrective Action:
All Valcor solenoid valve coils, S/N 738A to 745A and 746 to 807, were replaced.
The new coils are clearly identified and controlled by production batch number.
The batch number will be marked on the coil and will be listed in certification documents.
