Dear Mr. Keppler:

In accordance with Reference 1, this is to inform you of a personnel error which could have prevented the fulfillment of the functional requirements of a system used to cope with accidents analyzed in the Safeguards Report.

It was concluded on August 6, 1980, that the existence of a 1/2" angle type globe valve, found in the closed position on the Shut-down Condenser shell, created the potential for degradation of the containment vessel boundary.

The addition of the valve cannot be traced to an authorized design change.

The valve was replaced by a pipe cap on July 21, 1980.

The installation of the cap returned the system to its original design which had undergone numerous Containment Building leakage rate tests in the past.

installation had also undergone satisfactory containment leakage rate tests in the past.

Leak tightness of the recent cap install-ation will be accomplished today.

Prompt notification of this event was made to Mr. Ken Ridgway of Region III, U.

S. Nuclear Regulatory Commission, at 1445 on August 6, 1980.
