Gentlemen:

This refers to the inspection conducted by Mr. I. T. Yin of this

office and Mr. R. E. Shewmaker, Office of Inspection and

'

Enforcement, Headquarters, on June 24, 25, 30, and July 1, 1976,

of activities associated with Construction Permits No. CPPR-81

and No. CPPR-82 at the Bechtel Ann Arbor office, and at the

Midland plant construction site, and to the discussion of our

findings with Mr. Southworth and others of your staff at the

-

conclusion of the inspection.

The enclosed copy of our inspection report identifies areas

examined during the inspection. Within these areas, the

inspection consisted of a selective examination of procedures

and representative records, observations, and interviews

with. personnel.

No items of noncompliance with NRC requirements were identified

during the course of this inspection.

In accordance with Section 2.790 of the NRC's " Rules of Practice,"

Part 2, Title 10, Code of Federal Regulations, a copy of this

letter and the enclosed inspection report will be placed in

the NRC's Public Document Room, except as follows.

If this

report contains information that you or your contractors

believe to be proprietary, you must apply in writing to this

office, within twenty days of your receipt of this letter, to

withhold such information from public disclosure.

The application

must include a full statement of the reasons for which the

information is considered proprietary, and should be prepared

so that proprietary information identified in the application

Power informing the IEC about the proble=s.

Infraction 45

"10 CFR Part 50, Appendix B, Criterion XVII requires in part that, ' Sufficient

r cords shall be maintained to furnish evidence of activities affecting quality.

The record shall include at least the following:

Operating logs and the results

of reviews, inspections, tests, audits, monitoring of vcsrk performance, and

materials analyses.... Inspection and test records shall, as a minimum, identify

the inspector or data recorder, the type of observation, the results, the accept-

ability, the action taken in connection with any deficiencies noted. RecorJs

sha*L1 be identifiable and retrievable....'

" Contrary to this requirement, the results of reviews, interim inspections and

monitoring of reinforcement steel installations were not documented with regard

to the type of observation or the findings, nor did they contain the action taken

in connection with any deficiencies noted."

Comments

-

At the time of the 130 inspection, Bechtel personnel had been exceeding the

requirements of their program by =arking up copies of drawings with rebar

,

requirements during their in-process inspections. These marked up drawings

then were documentation of " interim inspections and monitoring of reinforce-

-

ment steel ins' h tions." In response to this " infraction" Bechtel Q"Hty

Control has modified its program to require this activity to be perfor=ed.

-

The point is that the activity was being perfor=ed, however, on an "infor=al"

basis. The effect of the " infraction" has been to formalize this activity.

This infraction relates to the lack of documentation for interim inspections.

The Bechtel Que.lity Control progr!u:1 has also required a final inspection and

the documentation of this inspection and this has been perfor=ed.
