Midland Proceeding - Tendon Sheathin_

Gentlemen:

On April 19, 1977, a nonconformance involving the placement of four tendon sheathings to be installed at approx-imately elevation 700'-704' in the Midland Unit 1 containment building was discovered.

These four tendons were to be deflected downward to clear the two main steam line pene-l trations at center line elevation 707'-0".

Contrary to this, two tendon sheathings (H32-036 and H13-037) were installed f

horizontally and two tendon sheathings (H32-037 and H13-037) were not installed.

Concrete was poured to a construction joint at elevation 703'-7" approximately one week before this nonconformance was found by the. survey team making preparation for the next wall lift. shows the design and "as built" alignment.

The NRC was notified on April 19, 1977 of this nonconformance and NRC personnel inspected the l

jobsite on April 21 and 22, 1977.

At the present time, Consumers Power is proposing below the penetration and above the 703'-7" elevat d(H13-037) to correct this matter by installing one sheathing n and l

one sheathing (H32-037) above the penetration.

Th remaining two tendon sheathings will remain as placed.

Attac ent 2 illustrates this proposal.

This proposal has been ap, oved by the architect-engineer and the supplier of the post-tensioning system and meets the original design requirements.

This proposal was reviewed with the NRC on May 4, 1977.

There will be no delay in the schedule and the cost will be less than $30,000 to implement this proposal.

s With regard to the cause of this incident, the drawings and survey layout control points were correct but they were not followed when the sheathings were installed.

To insure that this nonconformance is not repeated, th'e survey crew will be required'to verify in writing the correct placement of each sheathing, rather than verifying the in-stallation of all sheathings in a wall lift collectively, as was the prior practice.

The Quality Control Instruction has been revised to emphasize that Quality Control personnel are to compare the actual-installation to the drawings and to verify the configuration and number of sheathings.

Consumers Power has also agreed to implement the six action items recommended by the NRC in its letter of April 29, 1977.

Discussions on corrective action are continuing between Consumers Power Cand the NRC.

I will provide the Board and the parties with a copy of the final report on this matter when it is completed.
