Grand Gulf Nuclear Station Bechtel Job No. 9645 File: 0260/0512 10CFR-Part 21 Repor: ability On April 24, 1980, Bechtel Power Corporation, the Grand Gulf Project , verbally notified Mr. Floyd Cantr ell of your Of fice of a 10CFR Part 21 deficiency at the Grand Gulf Nuclear Station Construction site.

The deficiency concerns Rose =ount Model ll51A Pressure Trans=1::ers with a characteristic that the ou:put may read within nor=al range when the input is under- or over-ranged.

We are currently investigating this matter and will report our findings to you within 90 days (July 23, 1980). Details are described in the attached 10CFR Part 21 Report.

Nature of Deficiency or Failure Rosemount Model 1151A Transmitters , incorporating "A" electronics , are specified to have a linear output of 4 to 20 MA throughout the calib-rated range. ,Rosemount has observed, in a limited nu=ber of transmitters, that when over-ranged at 1407. of the instruments upper calibrated limit, the ou:put current will fall below 20MA. Si=ilarly, if the trans f::ers under-ranged cet - tion exceeds 1907. of the calibrated span, the tr'ans=itter cutpui may be greater than 4 MA.

An evaluation of these transmitters in a safety related system has shown that the deficiency in a transmitter would adversely affect the operation of the system and provide erroneous information which

=ay cause an operator to take i= proper action.

The Date on which Infor=ation of Deficiency was Obtained The date on which inf ormation of deficiency was obtained was April 1,1980.

A detailed review was concluded on April 22, 1980, which determined that received componen:s are, in fact, utilized in safety related systems.

  • gr-10CFR Part 21 Report Page 2 of 2 Since that ti=e, all Bechtel Power Corporation associated Nuclear Power Plant projects have been contacted by telephone and advised verbally of the Rosemount Model 1151A deficiency and probles and were asked to deter =ine if any Rose =ount ll51A transmitters were in safety related service in their respective plants.

In addition to Grand Gulf Nuclear Station, responses indicating applica-tions of the subject Rosecount 1151A Transrteters in functions where the deficiency eculd adversely af fect safety, have been received f ros:

Bechtel Power Corporation -ill be responsible for the corrective actions to be taken for all plants in their control.
