Appendix A NOTICE OF VIOLATION Illinois Power Company Docket No. 50-461 Based on the results of tne NRC iapection conducted nay 1-30, 1980, it appears that certain of your activities were not conducted in full compli-ance with h3C requirements as noted below. This item is an infraction.
10 CFR 50, Appendix B states, in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings.
. and shall be accomplished in accordance with these instructions, procedures, or drawings."
The Illinois Power Quality Assurance Manual, Chapter 5.B.1 states:
" Written procedures, instructions, and drawings shall be developed and used, as appropriate, for activities affecting quality."
Baldwin Associates Quality Assurance Manual paragraph 5.4.2 states:
" Fabrication and Installation of Installation of Items, Systems, and Components" purpose is to provide instructions for work involved on Safety Related Systems. Paragraph 4.1 states: "The Senior Discipline Engineer shall be responsible for.
. issuance of the traveler packet which defines and documents the work to be performed.
Contrary to the above, a three inch hole was cut in the Reactor Building dry wall without approved prescribed procedt'res, traveler, or design change.
