Occurrence Date: July 8, 1980 3.
Facility: Trojan Nuclear Plant, PO Box 439, Rainier, Oregon 97048 4.
Identification of Occurrence:
The plant service water effluent pH and chlorine exceeded the Technical Specification limits.

Conditions Prior to Occurrence:
The plant was in Mode 5, Cold Shutdown at the time of this occurrence.

Description of Occurrence:
During chlorination of the service water system, more chlorine was added than required and this resulted in exceeding the pH (lowest pH was 4.5) and chlorine (highest residual chlorine was 4 ppm) limits of the Environmental Technical Specifications. The limits were exceeded for approximately eleven minutes total.

Designation of Apparent Cause of Occurrence:
The cause of this occurrence was personnel error. Approximately ten times more chlorine was added than required to chlorinate the service water system, and the capabilities of the sulfite chlorine removal system were exceeded for a short period of time.

Analysis of Occurrence:
This event had no effect on plant or public safety and had no effect on the Columbia River. The high volume of flow associated with the river would quickly disperse the
-all amount of chlorine and negate any potentially harmful effects.
9.
Corrective Action:
The immediate corrective action was to stop the chlorination, increase the dilution flow and add more sodium sulfite to remove the residual chlorine.
The permanent corrective action was to reinstruct the personnel involved.
