APPENDIX A NOTICE OF VIOLATION Tennessee Valley Authority License No. DPR-77 Sequoyah Unit 1 Based on the NRC inspection April 8-30, 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
As required by 10 CFR 73, paragraph 73.55(d)(7), all points of personnel and vehicle access into vital areas will be positively controlled. Section 5.2.1 of the Sequoyah Nuclear Plant Physical Security Plan requires in part that all vital areas barriers be resistant to penetration, and that the penetration resistance of the barriers will not be diminished by doors, windows, and other openings in the barrier.
Contrary to the above, on April 12, 1980, the inspector found six (6) unpro-tected openings, approximately 18 x 24 inches in size, at ground level in the barrier wall of the unit I steam valve instrument room (west), a vital area.
This is a deficiency.
