1.0 INTRODUCTION
An inspection of the Salem Generating Station was per-formed by Mr. Walter Rekito, Reactor Inspector, from the Nuclear Regulatory Commission Office of Inspection and Enforcement, on April 22 thru April 25, 1980. The NRC Inspection number is 50-272/80-10. The inspection included a review of the records, procedures and data relevant to the Reactor Containment Leakage Test Report. The results of the inspection were reported as follows: One item of noncompliance (Deficiency Level) and one unresolved item pertaining
- to the ILRT of Unit 1.
This addendum to the ILRT report is-the response to the inspection and will, upon submittal to the NRC, provide the data required to correct the item of noncompliance, and the data and analysis necessary to resolve the unresolved item. The specific inspection items which are the subject of this addendum are as follows: A) Item of Noncompliance - Deficiency Level

The report failed to include all Type C valve tests performed since the last Type A test, in the B&C Test Leakage summary.
(O x' The data pertaining to these eight tests has been retrieved and a new summary sheet is included showing the total leakage to date. An analysis of the Type C test of Penetration 19, Valve IVC 2, is included. This valve was the only valve not in alignment during the Type A test and was included in the Type C Leakage added to the ILRT along with Penetrations 40, 61A and 61B. The Test Data sheet and the change in the procedure master (Check off 4.1, page 6) are also attached to this addendum. B) Unresolved Item The volume of water contained in the reactor containment sump and reactor coolant system were not evalua'ted upon conclusion of the Type A test. A detailed review of all available records was conducted and an evaluation was made of the contribution that each system had on the results of the Containment Integrated Leak Rate Test. The evaluation showed that the reactor coolant system did not have an affect on the ILRT. The containment sump level did have an affect of significance to change the ILRT results, however the new figures 1 do not exceed the acceptance criteria of 0.1%/ day (a .

" Type C Leakage to be Added" section of Table 1, because the valve closure had not been " accomplished by normal operation and without any preliminary exercising and adjustments...."
(10CFR50 App. J sect. II A (b) . ) The valve IVC 2 had been removed for repair of a binding chaft just prior to the ILRT. (Work Order O.D. 905618) - Subsequent re-installation of the valve could not be completed, in that (') the control air lines connections and retest were not possible after the control air system penetrations to the containment had been isolated and vented as a prerequisite of the ILRT. The valve body was received from the repair facility in the closed position and installed in the line without further question, just prior to commencement of the ILRT. Therefore, the reported leak rate total would have to be increased from 0.056 to 0.062% per day and at the U.C.L. t from 0.062 to 0.068% per day following the intent of the procedure. However, it is believed that most of this water entered the sump from continuous leakages of service water at various valves and joints inside the containment. Service (~.% kr' water was in operation to support running the containment fan coil units. The aggregate of these known S.W. leaks was variously estrmated at one or less gallon per min. by different inspectors. The time period between sump inspections (0600 on 8/9 to 1200 on 8/13) is 102 hrs. so the indicated leak rate to the sump is: L g, ,

, add the above figure, as an additional penalty, to the
" Type C Leakage to be added", Section 7.1 Table 1. This raises the total " penalty" to 898 sccm, or about 0.0016%
per day. Therefore, the total calculated leak rate is adjusted (rounding off to three significant digits) as follows: ; L,,= 0.058%/ day UCL = 0. 06 4 %/ day 4.2. Fressurizer Level Check off sheet 4.7 records Pressurizer Level at 2% before the test, and at 82% after the test, as read on the Cold _ calibrated level instrument. Fortunately, data could be 1 le I retrieved from operators logs, showing that pressurizer level remained constant during the execution of the measure- .F ment program, at 2% indicated. During pressurization, the control room operator found it necessary to charge into the RCS frequently, in order to keep pressurizer level indication above zero on-his instrument. This was because as the containment air pressure increased, the air trapped at the top of the Steam Generator U-tubes was compressed. This, of course, was due to the water level in the pressurizer seeking static equilibrium with the water level in the steam generators primary side. P O

/ On depressurization, water surged back out of the U-tube bundles, raising pressurizer level to 82% (cold calibrated) before the' operator took action and initiated low pressure letdown via the RHR system.
See the attached graph of containment pressure and other significant events which occurred during the ILRT. The attached summary of excerpts extracted from microfilms of various operators' logs and Test Engineers' data sheets is included for clarity of events as shown on the graph. Other tanks of interest include the VCT, RWST, RCDT, and PRT. All of these tank levels were logged periodically
; during the test by the operations staff, and all remained constant.
It is concluded therefore, that no water inventory change took place in the RCS or connected systems during the time of the reasurement program, and no additional corrections to the measured leak rate are required. I I0 p i-~- - -- - v-
